Good morning LantusLand! Yesterday's Blues

pm +10 - 345
AMPS - 307

I start off this morning with a question. I tested KT at pm+10 - number was 345. I fed him 6% carb Salmon, about 4 ounces, at pm+11. When shot time arrives, I test again (2 hour span). This time he gave me a 307. That's almost a 40 point drop WITHOUT any insulin. I know he's bouncing in these high numbers but

QUESTION: WHAT would cause this drop with no insulin support?

Hi Lyresa,

You know I'm not an aswer person, but this happens all the time with Weezer. Don't know why. Don't forget the 20% built in variance for any test result.

I can't see thhe ss due to firewall at tbp, however it could just be the bounce clearing, or a little sputtering of pancreas action (ideally food stimulates the pancreas to produce insulin) or a little of both.

Oh how I would LOVE to have that pancreas sputtering back to life!!!!!! I'll dance 24 hours at a time if that has a chance of happening!!!!

It could be any of the things mentioned....remember meter variance...that is well within the 20%.

Is there some reason you fed an hour before AMPS? It's best to withhold food for 2 hours before PS to avoid food influence. (No I'm not the food police, but feeling like it lately.... ;-) )

And let me throw in that it could also be the second lantus dip. But in reality, I agree that those numbers are pretty flat with meter variance.

My thinking was with a 345 at +11 which WAS a pre-food number, his AMPS would definitely be high enough to shoot regardless of whether he ate or not. Since we dose on nadir, I wasn't going to change his dose because of whatever number I got. We got off schedule when we stalled a couple of days ago. We feed 19 animals before we feed ourselves each morning - the 3 'main house' cats being the first bowls down....they have to finish their food and bowls picked up so the puppies can eat. If I put off feeding KT, that means no one gets fed inside for that time - he'll eat everyone else's if he doesn't have his. Even the 12 lb. puppies back off when 17 lbs of cat gives them the 'eye'...

Please help me understand where my thinking above needs to change....my 'logic' isn't always logical... :roll:

The other reason to not feed late in the cycle is it slows the last effects of the insulin down and can result in higher PS than normal. In other words, if he does get a second lantus dip, it can preclude that.

For today, again, I don't really see a drop in numbers. 345 and 307 are virtually the same number. BUT in the future, feeding between +10 and +12 can preclude any second lantus dip and possibly result in a higher PS than you would have gotten normally. AND...as Laurie mentions, it can also influence the PS with a food spike so you aren't shooting a real number. Not such a big deal at high numbers but definitely a big deal at lower numbers as it gives a false sense of security. If, for example, he was 70 at +10, you fed him at +11, and he was 100 at PS, that would be a food spike. If he came DOWN after PS instead of rising, you could be fighting low numbers later on.

The reason I didn't think it would matter today WAS because of the high numbers. I would handle it completely differently if he were even in the high 100's....you all have already taught me those steps and they DO include:

* no food in AMPS.
* not much food at a time when trying to bring low numbers up.
* letting HIM try to pull them up at the end of the cycle by himself rather than artifically inflating with hc panic feed. If he needs food, make it lc...
* little mc will slow too fast of a drop (but KT dives off cliffs...)
