Hi everyone. We haven’t posted since 10/7 and it seems I have lost regulation for Minner.

I don’t know what to do so I’m looking for ideas.

Since around mid August, yellow readings started to appear. It led to me getting her vet checked (labs posted in sheet) and then a dental. Yet her levels have continued to be unstable and more & more yellow. We even had one pink in last 2 weeks, the first one this year. Sigh.

I know this may not sound terrible relative to what others are managing. It’s more about the change. Why is it changing.

We are on cycle 3 of no green right now. Yet 4 cycles ago we hit a 62. But overall if you glance at her sheet, you will see that yellow is showing up a lot more.

Sigh. I know when we’re over 5 units, eligible increases (6 cycles no green) are recommended at 0.5 instead of 0.25. If Minner has 3 more cycles of no green, is it ok to increase to 6.0 instead of 6.25, or is that not recommended because of her IAA? I’ve wondered if tweaking small increases actually is a problem b/c it allows the antibodies to catch up vs be smothered. Who knows. This is super frustrating.

Minner so far (knock on wood) seems fine. She pees a little more in yellow and I think she sleeps a little more (less active).

I appreciate any and all thoughts on her situation based on what you see with her trend in her sheet.

Thanks so much.

I used the rule of thumb, increase by 0.5 units if over 5.0 total units, unless just tweaking a dose showing some green, in which case you do the 0.25 units. Meaning, it's OK to go back on the syringe line at 6.0 units. Of course, that depends what she gets down to tonight..... I noticed you had done a 0.125 unit increase. I found the larger increase better along with smaller decreases.

Did the vet check after the dental to make sure no infection?

Thanks Wendy. I look silly. Look at that nice 72 nadir for this cycle. Hopefully not too low to prompt a bounce, but early enough in the cycle to maybe need a bit more carb to not drop lower so I gave her a tsp and a half of 1% after the 72 reading. But she still might bounce up to yellow by AMPS. This never used to happen, these wide swings.

There was no follow up to vet after the dental. She got convenia injection. I will get her back in to take a look. Is this something checked visually, or also bloodwork?

Visual check is done to see if infection. If she's eating fine, she's probably OK.

I found with my IAA kitty that I had to really keep tight reign on her numbers. Or else she would get bouncy fast. But would eventually get back to where she should be with the enough insulin.

Good to hear from you Laura, sorry about the weird yellows...

I would echo what Wendy said about the bounciness on just slightly too small a dose. Bell has had her best run in awhile on this 5.5 dose, which seems a wee bit strong for her, but is seeming to keep her from bouncing 50-70% of the time. It is just such a tight rope to walk.

Thanks so much Seth for the additional feedback. Amen on the tightrope. Very good to hear Bell is having a good run. She looks great!

I may tweak Minner up a little. I was worried that my non traditional minor tweaks might be counter productive with IAA kitties, like you need bigger changes to overcome antibodies … kind of like not properly taking antibiotics in large enuf dose or long enuf and then infection comes raging back stronger …. but I am just making that up in my head about IAA, I am aware of no science to suggest that antibodies get fiercer or stronger if you don’t aggressively over power them.

I should post more too. I forgot how it helps alot to just be able to air this out a bit with fellow feline friends. :)
