Hi there!
Not quite sure if I should give Aida her regular 0.5U, she hasn’t eaten, she’s been off for the past week now with some days a bit better, mild diarrhoea, and this morning no appetite. She just left her bed to poop a very smelly and mushy turd (she’s chronically constipated and I usually give her miralax to help, obviously I haven’t been giving her any lately)
Any advice welcomed!
Also, I’m trying to get hold of the vet to bring her in today. It might be that the IBD the internal vet saw on the ultrasound in May is starting acting up?

She ate at +6, their usual middle of the night-snack. Since then nothing, I offered all sorts of things, the toothpaste she absolutely adores was also not interesting…

Yes, no interest in food, she doesn’t seem uncomfortable though, she’s looking at me typing this down with her smiling eyes.
Giving her 0.25U
Many many many thanks Bhooma!

She still hasn’t eaten, I’m going to buy some RC recovery and Hills AD just in case.
Got a vet appointment for Monday.

Should I remove the question mark from the title?

Ok, she just ate 14% HC canned, a tiny amount maybe 30gr?
I give them 250mcg B12 bid, so 500mcg daily.
I used to give them saccharomyces boulardii but it constipated them both, and Aida usually needs help to poop so I stopped it. I still have a bottle in the fridge.

She doesn’t look nauseous, or uncomfortable, or in pain. I usually can pick it up when she has flares from arthritis, her face is completely stressed but now it’s not the case, she lies there, opens up her eyes to look at me and smiles. I might be misreading it, not very good at the feline grimace thing…

Should I go get some Ondansetron? I do have some maropitan.

It could also be pancreatitis but IBD sounds very likely. She’s definitely nauseous still so as Bhooma suggested ondansetron might work better. It has for all my nauseous cats. Is it available where you live? It needs to be given at least 3 times a day.

What did you do to treat IBD? I would change to a protein new to her.

Somehow I has mistakenly understood or misread the reverse between maropitan and ondansetron!

I have some cerenia left from France, but here it’s not on the market (although officially it is sold in all Europe, each country has its own agenda).

I was able though to get hold of a box of ondansetron, 18 pills of 8mg, no prescription needed (!). Teh pharmacist told me that there’s currently a shortage of this medication and it would be very difficult to find the 4mg.

My understanding is that it should be dosed at 1mg bid? Or 0.25mg/kg?

Aida has never officially been diagnosed by any vet with IBD, it’s just the ultrasound vet that saw the thickening of her intestinal wall and said either IBD or SCL, a biopsy would be needed to differentiate between them.
Her regular vet then didn’t want to do anything, since she’s been asymptomatic until now.

On Monday we’ll do a CBC, chemistry and cobalamin+folate. It’ll be the first time that she’ll be getting a serious and complete blood pannel!

About novel protein is there a big name brand? It’s very limited here, foodwise, even more than in France, from what I’ve seen so far.
She’s getting chicken currently, sometimes beef and rabbit, should I try to find something like lamb?

Progress. That’s good. The ondansetron dose is 0.5mg per kg every 8 hours.
As for a novel protein I mean anything she has not yet eaten in the past. Lamb or pork might be good. Just know to see if that helps she would need to eat only the new protein for at least a month or two. One of my cats out of the blue was having severe diarrhea and inappetence. She had been totally healthy prior to my trying to feed my two cats the same food as her sister has allergies. I eliminated the two foods and that worked.

0.5mg that would be for her 2.5mg, a quarter of one 8mg pill, thanks!

Thank you for clarifying that with the novel protein! I should probably start cooking again for them, haven’t looked into it… To be honest, it’s so convenient with the canned food! But also frustrating to see the low quality and lack of care in the formulation…

Pancreatitis episodes are not uncommon with IBD, or SCL. Constipation too can go with inflamed bowels.

If you are supplementing B12 now, the cobalamin test won't tell you if she's low. Might want to do that after she's been off B12 for a couple weeks.

Oh no… that means that it’s pointless to do the cobalamin and folate test, right? Since they’re codependent?

ok now she ate her breakfast. She ate her night snack as well. She pooped a somehow less smelly turd, I guess the boulardii takes effect.

And now its the AMPS moment, she was at 84, my understanding is to retest in 20min and if her BG goes up to shoot the full 0.5U since she’s eaten, correct?

Thank you all so much for your invaluable insight and guidance!!! I’m so grateful!!

It's great that she is eating and her poop is less soft/smelly! :-)

If she is eating and you can monitor, you don't need to stall. You can give the full shot. Don't fill her up so that she will be willing to eat small amounts later in the cycle if her numbers drop.

I would give Bandit 2mg every 8 hours when she had bouts of inappetence (pancreatitis, kidney infection etc). I believe one of the side effects of Ondansetron is constipation. :nailbiting:

Noted, just gave her 2mg, she hated it… unlike Feloro she’s super difficult to pill, I might use that recovery food to pill her if it becomes a regular thing!
Don’t remember if I saw this here or on the CKD group, in case it was over there, a video with amazing tips to pill difficult cats:

If she gets constipated I’ll start her again on a tiny dose of miralax. She usually goes once a day, so I’ll see it easily.
