10/9 Shadow ambg 123 might discontinue budesonide

Morning all....
I tested Shadow last night and saw a 200. I checked twice.
This morning she started at 123.

I did not give insulin yet. We have a vet appointment in an hour to try to aspirate the flat lump on her side. And talk about whether a ct scan would give us more info.
I have a pen of insulin in the fridge.... it's been there a year and I believe it expires next month. Hopefully it's good.

I knew I couldn't stay up last night with her because of a medication I am taking. I won't take it tonight so we can start low.
I assume she will go up again after she gets her budesonide.

She was a little playful last night but only ate one of her servings of food for the night.
I remind myself that she just feels like herself.

She's going to be very upset in a few minutes when I put her in the carrier.
Last time she fought them trying to get blood after her u/s ....we had been there too long that day.

we're home.
Need a visit from the KTSS...... droolers must smell her stress..... they are following her everywhere.....
surprised she hasn't whacked them yet....


vet was able to grab hold of lump.... very spread out and flat but it doesn't seem attached.
she made two slides.... one to look at immediately and one to let air dry and stain later in the afternoon.

The one showed just fat/glycerin.....so looks like it's probably just a fatty lipoma but so was the last one.....a few years ago.
I don't trust lipoma's. They seem to always become cancer later on.

saw a different vet and had to fill her in on history....
the owner vet will call me back to tell me where and what I might choose to do.

IF we remove the lipoma, I guess he could do a little exploratory surgery.

I'm trying to convince myself that an endoscopy wouldn't be so bad....but it means one of those big vet hospitals where they won't let me walk past the flapping doors.
That's much more difficult for Shadow.
She does better when I'm with her staring into her eyes and scratching her head.
Those places also tend to want to keep them overnight.....

Did you ask about her BG? Also, did you try feeding and retesting 3 hours later to see if the pancreas kicks into action?

Fingers and paws crossed that it is nothing serious and that it can be dealt with swiftly. The KTSS is on the way!

Rhiannon, I'm sending lots of "good results" vines. My Tux had a lipoma removed, several years ago, and nothing has come back. Have faith! :YMHUG:

okay....my vet practice has a few vets so they "talked amongst themselves"
and agree with me....

they think it is more likely lymphoma than ibd since she has no ibd symptoms. No vomiting or diarrhea.

They haven't seen too many "good" results with chemotherapy. And said I could go that route but we would have to find out specifically what we are dealing with.
They don't really recommend I do that.
They are looking at the big picture.... Quality of Life ... and they know how Shadow responds to being at the vets/surgeries/etc...

Also the budesonide probably isn't doing her a lot of good since it's more for IBD.
Perhaps discontinuing that would keep her blood sugar in check.
He did recommend that we might do another u/s and see if there are changes happening.
The fact that we could see two lymph nodes by her spleen that aren't usually visable at all on the ultrasound isn't good.
The lipoma stained slide shows just fat...... no cancer cells..... yet.

However, it is probably related to the lipoma she had removed in 2010 ( an infiltrative lipoma which is extremely rare in cats) They had a hard time getting it all and took healthy muscle in the effort.
The location is still in the muscle surrounding the rib cage.... just on the side this time instead of under her sternum.

There also isn't going to be anything they can do for the pylorus..... that's a really small space and if the thickening continues.... she will quit eating.
So there is a big bridge to cross coming......
but I have no gps.....

The news is really the same as before.....so I'm dealing with it.
Just keep holding my hand everyone.....
who knows how long.?

Sending tons of prayers for you and sweet Shadow. Did they suggest any alternatives if you discontinue the budesonide? We're with you, every step of the way :YMHUG: :YMHUG: :YMHUG:
 
I could go to prednisone... but their opinion was it would only buy me a little more time...... that it just helps for a while.... and then it stops helping.....

in the total QOL picture..... I don't know....
is it better to have the prednisone on board and go back to giving shots for her diabetes....?



They seemed to think I might buy myself a few months but we'd be back meanwhile doing the sugar dance....
Tonight's bg test is 148....
They wanted me to wait a couple of days before restarting insulin and give the budesonide time to be out of her system.
So she is going back down....
and I'm- a- checking.
She had raw chicken tonight while I was cutting some up for dinner.

She's acting like she knows nothing..... :smile:
she even ran around the room while ago and played "gotcha"..... always dangerous for my hands....

((((Rhiannon and Shadow))))) I am so sorry to hear the vet report wasn't a good one - You have so many tough decisions to make, I don't have any information to offer but what I can do is send tons and tons of vines for comfort, calming and strength. :YMHUG: :YMHUG: You will both be in my prayers!!!

I've read quite a bit about lipomas and every single thing I've read said they are not cancerous and cannot become cancerous. However, there are also liposarcomas and those are malignant. Liposarcomas are also found in fatty tissue. So if it is really a lipoma, you shouldn't need to worry about cancer.

I'm not sure if lymphoma symptoms are ECID but we were pretty sure Gus had lymphoma and he had intermittent diarrhea and occasional vomiting. Some cats, like Linda's (Bear Man) Teddy responded very well to chemo and lived two more years after his treatment with a very good QOL. Our oncologist sho treated our Teddi for lymphoma said it depends on the type of lymphoma. Small cell lymphoma responds very well to chemo; large cell, not as much.

Do you think the budesonide is helping her? If you think it is, why take it away? If you have seen no change or improvement, I think it's reasonable to stop it and let her BG stabilize. If you decide to start pred, it needs to be prednisilone, not prednisone. Not all cats can convert prednisone to prednisilone but the latter is the form they need. You might have to get the prednisilone compounded as they don't make the pills anymore.

We have prednisolone pills that were compounded for Zener. They are called tiny tabs and are quite easy to cut in two, and also wrap in bits of pill pocket. He took them like a champ. If you decide for a trial of prednisolone, let us know. You can send a PM with your address and we can send you some to trial. It helped Z when we switched from Budesonside to prednisolone, an pd gave us another two months.

Rhiannon, I am so very sorry to hear the news from the Vet about sweet Shadow. You both are in my prayers.

I'm so sorry that the news isn't better. It seems most vets have a grim prognosis when lymphoma is discussed. Small cell is controlled usually successfully with a chemo pill and prednisolone. I know of cats that have lived longer than 3 years with it. My Tiffany had intermediate cell in the intestines. It acts much like large cell. She lived for about a year, most of it good. She had agressive chemo. They don't give enough to make them feel sick like happens to humans. She failed chemo after going into remission. She only had it for about 2 months because she developed a UTI that destroyed her already compromised early CRF kidneys and the oncologist let happen because she didn't test for it. Pred only kept it in check for another 9 or so months. It is true that you would need a biopsy to know what it is you are dealing with as the drugs used are different. Tiffany was very scared of the vet and leaving my place but she did okay with chemo. If in the situation again I would try chemo I think. Usually it's the cancer that makes them feel sick rather than the treatment. There are good nausea drugs to deal with any effects. Tiffany was only 11 when diagnosed . If you don't do chemo and I totally understand why you might not, pred might make Shadow feel better. While not a cure it might give her some more time with quality of life. I'm so sorry.
