So I thought he was settling into his new dose well already, thought wrong. Still on the bounce, hopefully he will come back down today. He woke me up at 4:30 this morning licking my face and I had to point to the food in the feeder (was already open) for him to dig in. He uses it no problem when we are not home though. At least he was feeling well enough to climb into bed and lick me instead of just wailing beside the bed. Found puke at the 4:30 timepoint. Not much though considering how much the kid eats. . . .

What would you think about raising Buddy's dose tonight providing nothing changes this cycle? It looks like you've got room to increase by 0.25u.

Already considering it. Wondering if I should wait till tomorrow morning or night so that I can be home with him Sunday once the full effect of the increase hits. So are you talking a full 0.75units? or more?

Also, I had one question. Something occurred to me on the way to work today. We've talked a lot about the "shed", and I've read a lot on the new to lantus links about the "shed" as well. Using this metaphor, at least in my mind relates to insulin that just always has to be there first, to be able to see any effect - almost like it's not bioavailable. I've been using the assumption that somehow the duration of the insulin in cats is only 12 hours give or take. I believe I've been really stupid about this idea and would like you to weigh in on this. The duration is probably more like that in humans - 24 hours. The reason why we see "12 hour cycles" with BID, is that in reality there's really 12 hour overlap. So it takes 2 doses to get to "full" dose, ie. 0.5uints BID is sorta like having 1unit at all times. EXCEPT: it is Better because there is always that 0.5unit sitting there - you guys call it the shed. So that the cat is never really going without insulin. I even wonder if humans would actually benefit from dosing the way we do. Pharma has a knack for pushing the limits and having less often doses, cause that's what the consumer wants. But humans are different than animals in the way they eat, when they eat, and as far as cat's the amount of Glucose they produce in the liver as a normal function. Please let me know if you think this discussion will be a bit confusing to laypersons and it should be edited out. It's still such a shame that there is no PK data from Sanofi in felines. Would be a wonderful idea since so many of us are using it.

Great link! Not 100% convinced as to the mechanism of it all, that it is a faster metabolism thing as the article even mentions Lev as a possible exception. 99% convinced BID is the best way to go with most cats and some require TID. I'm sure there are a small number who actually do better SID as ALWAYS and NEVER are too strong of words to count SID out entirely which is why it is still at the back of my mind should I not see further improvement over time in Buddy. This is definitely a longer timeline kinda thing, just not ruling it out completely for the future should I need to try something else for him. No matter what the reason is, I'm staying the course with BID for a while. This was all just a purely scientific question here. So wish we could get PK studies done. Wish I worked for Sanofi instead of BMS. . . someone might actually listen to me and do the studies. They could probably make a good deal of money if Lantus was labeled for use in Dog and Cat.

Oh, and what do you think of a new dosecrease? 0.75? 1unit? Should I put it off for a cycle or two to be around once it takes full effect?
