Good evening,

so far I managed to poke Binie two times each cycle, one before the shot and one mid-cycle. Binie is in a good mood, which makes everything easier. Today my neighbor took care of her when I was away and I rushed in at lunchtime to do a test. Everything went well. I gave her high carb in the morning, maybe that was not necessary and so she had three pink numbers today. :banghead:

And we got the test results from the urine test. Unfortunately we are out of luck. The multi-resistant germ is still there, still in high quantity. Which I find so strange because the germ is sensitive to the last antibiotic I gave her. I thought it would at least reduce the germ? I was really super strict about giving it to her every eight hours, Binie was so brave with all the side-effects and it really helped her not at all?

The young vet sent me the results by mail and asked me what I want to do now. I wish she could tell me. This all feels unreal, as Binie was really better the last days. Seems like the older vet is reading the mails in her holiday and I hope she gives advice.

I’m glad everything went well today. Good job with testing Binie. I know it must be hard, but sounds like she’s been ok. Good girl!!
I’m so sorry to hear about her urinalysis results. What does the vet’s office suggest you do now? Why are they wanting you to make a decision? What options are they suggesting??

I’m sending you prayers and love, Heike. Please let us know what they are saying you can try to do with her as a next step.

I've not been on the Board for a few days. It sounds like the vet did a culture and sensitivity to determine what the type of bacteria is. If this is the case, the vet shouldn't be asking you what to do. The test identifies the type of bacteria so the appropriate antibiotic can be prescribed. However, it may be that the problem isn't a UTI -- it may be a kidney infection. If a UTI has been present for a while, the bacteria can move up to the kidneys. It can take a much longer course of antibiotics to treat a kidney infection. This is a link to information on these sorts of infections so you can further discuss with your vet.
 
Oh Heike I’m so sorry to hear this. I hope your vet has some suggestions of things to try. Maybe an ultrasound of her kidneys? The pet radiologist here requires anesthesia but my vet clinic can do it just with gabapentin (although I don’t know how Binie might feel about it).
I wonder if there’s something in her kidney (like a stone or growth or some other foreign body) that’s giving a place for bacteria to hang onto and hide.

Hello Heike! I am glad your day away went well yesterday, but sorry about the results.
Do they have more options, other antibiotics, or maybe 2 that would act together? What germ is it?
I hope the vet can give you a call maybe, and discuss what they are thinking.

Thank you so much for your support and kind words! I am sorry I didn't give more background information yesterday. We have a antibiogram and Binie is dealing with a multi-resistant germ. There are not many treatment options. Unfortunately Binie can't take the most promising, amoxicillin/clavulanic acid, because she gets severe side-effects after the first dose. We tried two times and it is not possible.

There are two other antibiotics, the bacteria is sensitive on. The more potent one is not approved in Germany but we could get it from Switzerland. That was the Chloramphenicol, she took for two weeks and which didn't do anything else than making her very miserable. Giving it for longer than two weeks is not an option, because after ten days it starts to become toxic in cats. It was our big hope, that two weeks would be enough.

The other option is Doxycyclin, which is less potent than the Chlorampenicol and which is another hazard on Binies kidneys, that's why we didn't choose it before. I am quite sure, the vet will suggest this one now.

We are not sure if Binie has an UTI or pyelonephritis because we didn't do ultrasound yet. The vet said, the treatment doesn't change so we didn't do this to Binie.

The vet told me she has cases like this more and more and with male cats she chosses bladder irrigation with desinfectant and was successful with it in the past. She says you can't do it with female cats but I didn't ask why so far. And if one could, I wonder if we should.

I am coming more and more into an ethical dilemma. I want to give Binie all the chances and I would do anything for her. On the other hand I saw what all these treatments did to her and I don't want to make her quality of life less and less without any positive outcome. I did the surgery and it didn't help her but gave her a terrible time while recovering. I gave her antibiotics since many month and it doesn't help her but she felt seriously miserable on the Chloramphenicol, the other ones she seemed to do well with.

Right now, she seems to feel comfortable. She is outside on the terrace and even though she is still licking her breast and has these bacteria in her bladder, she seems to enjoy what she has. I know this will change if I don't treat her issues but if I start these treatmens or drive her an hour to the next good clinic for further diagnostics, I make her miserable for sure. I haven't made my decision yet and I am still in a process. I hope you don't judge me or think I don't advocate her needs. I just try to find a balance between her quality fo life and the duration of her life. It's a tough decision but I know it is my responsibility and no one can do this for me.

I’m sending you hugs and love. I know you are weighing every possible option to care for Binie. This is such a difficult situation with so many things going on. And the vet isn’t a great communicator so that makes it even harder on you to try to make decisions.
You are doing all you can to gather information to make decisions to give Binie all of the best possible treatments.

I am certainly here to lend support to you and Binie. Sending much love and hugs to both of you. Please give Binie lots of love for us and so glad she’s been enjoying her day on her favorite terrace :bighug::bighug::bighug::bighug:

Whatever you choose it will be the right decision—no one lives their kitty more nor has worked harder to help. There is a reason hospice care exists for humans—sometimes just making days comfortable is so much more than being miserable for dubious benefit.

I wonder if it’s difficult to see a female cat’s urethra in order to catheterize it for bladder irrigation —that’s the only reason I could imagine doing it only on males—but surely there’s a way
