I need help on understanding Dolly’s numbers. Flat for days, increases don’t seems to change anything. I’ve never been here before with Dolly. I fear the antibiotics are not helping. Just started another increase today. Am I being impatient? Is she possibly insulin resistant? I know this is sort of a stupid question since her numbers are somewhat controlled but unchanging. I need to know what to do, is it simply finding the right dose? Is it the underlying possible illness, chronic rhino-sinusitis (treatment is steroids), viral infection, cancer, fungal infection, nearly $6000 in diagnostics, not treatment. Do I try a treatment such as the nasal steroids that “can” work for 4 months? I am not willing to put Dolly through anymore than necessary. I will not get a second opinion, places are too far away and I don’t doubt this doctors diagnosis. Dolly does have an irregular heart beat so she may not be able to have anything done that requires anesthesia. I need to understand if we just continue to increase insulin as required. What would you do?

She’s only had the antibiotic for 3 days. I would evaluate after a full course. Is that 7 or 10 days? Your notes say the congestion is better. Is that still true?

Hi Elise, the congestion is better as far as eating although her diet has totally changed to fishy food, foods she would never eat in the past. The course for the antibiotics is 5 days then every 72 hours. She is eating well, no Mirtazapine. I’ve never seen such basically flat numbers but I am grateful they haven’t gotten any higher.

I honestly don’t know why her numbers have increased. It could be due to her issues but also she might have a little glucose toxicity. Hopefully one of the others can help you more. :bighug: I am glad she is eating.
 
I don't think this is glucose toxicity. She hasn't been on the recent dose overly long. Although, earlier on, you held the 1.5u dose for some time (8/27 - 9/22). I suspect the yellow numbers are related to how she's feeling. If there's an infection, give the antibiotics some time to work. Do you know whether these ABs tend to upset Dolly's stomach and if so, you may want to start probiotics. Just give the probiotic about 2 hrs prior to the AB.

She's probably eating the fish flavors because they are smellier. You could also try to put the water from canned tuna or the oil from sardines on her other food..

if the numbers stay elevated, just keep systematically raising the dose. If she does need steroids, it's not the end of the world. There are many cats that need to be on steroids for a variety of reasons. We work the insulin dose around whatever the medication is that may raise the insulin needs.

Thanks Wendy. I don’t know but I don’t think this antibiotic upsets her stomach. I have Boulardii, I could give it to her just in case. The steroid would be prednisolone and that would be to treat chronic rhino-sinusitis. She would need to go through probably a couple months of cultures to determine whether she has viral of fungal infection. She has had a culture not too long ago and they found pseudomonas, very hard to get rid of. Then they want to do a ctscan with biopsy but she would first need an echocardiogram to determine if her heart could safely withstand anesthesia. It is extremely expensive and that would just be diagnostics not treatment. I asked her what is the most important thing to do snd she said it depends on what will cause her demise first. I think I will try the nasal steroids that “might” help her. I am lost and scared.
