No great answers given at the vet. Regarding his ears, she recommended that we try the oral mirtazapine even though it does have more side effects to give his ears a break. Worried to change things up but his ears are definitely in need of a break. I'll fill it today and maybe give it a go tomorrow and continue with the mirataz tonight.

For his glucose numbers she didn't have much advice. She mentioned the "somgyi effect" and asked how often I test because he could be going too low. I told her she would think I am insane if I pulled out my spreadsheet so it isn't that lol. I brought up a dental and she said it is needed because of the amount of plaque but felt reassured that he isn't showing any obvious signs of issues. Before a dental she recommends seeing a cardiologist because of his murmur. Very complicated and expensive and I hate putting him through so much vet stuff. We'll see what the blood work shows.

Hi Albert, let's hope the oral mirtazapine doesn’t cause too many side effects for Xander and he eats well to take it. Gladly, he might only need it temporarily, just until his ears get a chance to heal 🙏🏼

An ECG with a cardiologist usually doesn’t take very long, right? And it’s generally not too uncomfortable for them. I think Xander would probably handle it better than we might expect. It’s a pretty non-invasive procedure, and with some gentle handling to help keep him calm, it could be easier on him.

How does he handle the vet visits?

I’m also glad there are no plaques and nothing serious going on in his mouth. 🙏🏼

And as for the BG, well, we already know it’s best to stick with the information we have here… as usual. But jokes aside, maybe with these next steps, his insulin needs will decrease. Let’s see if the lab work reveals anything else.

Let's hope he will handle it all well 🙏🏼!

Xander is so hungry tonight, as soon as we got home from the vet. Gaba really makes him want food.

I just picked up the script for mirtazapine 7.5mg and it is sooo small. Converting from the mg equivalent of the small amount of mirataz I give to the mirtazapine, his dose would be 0.005g of this pill 😮

Don't tell her what Jude is on (4 units!!).

Had Xander gained weight? I think that was Jude's issue, so now he's on a...shhh *spells* "d i e t" 🤫
He actually lost 2oz, pretty much steady. I can imagine how hard a diet is with managing the curves as well 🤗

Flare was also 20lbs at one time. It took a ridiculous amount of time to get him down to where he is now at around 17.

Also she also said 1.75 is a high dose and cats are rarely on doses above 2 units
🤣🤣 That's hysterical. I presume you've looked at spreadsheets of other cats around here. Neko got up to 8.75 units, and that's not high for an acro.

For the mirtazapine pill, back in the day before Mirataz, I've seen people refer to the dose as a "crumb". Some people preferred cyproheptadine as less side effects.

Has Xander ever had a CardioPet ProBNP test done? It's sort of a cheaper screening tool for the heart. Some murmurs are benign age related. Neko had several echos in her time, the first time it was just a benign murmur. Certain breeds heart issues are more common plus also for acros so double the reason for her to get an echo. I'll always regret an echo I delayed getting in another cat who had a murmur. After his first echo I was supposed to schedule a second echo for 6 months later. I delayed, finally scheduled it several months later. He passed from thromboembolism two days before the follow on echo. It could have been detected and the cat put on meds to prevent it.

Yes! I have to pick and choose where I hold my tongue to not sound like a know it all. That sticky regarding somogyi was actually the first thing that popped into my head when it was mentioned.

For the Mirataz, I give a pretty small dose of about 1/4 of the 1.5in strip, which I estimate to be around 0.5mg of mirtazapine, so im going to aim for around that dosage in weighing the pill dosage. Hopefully not too many side effects as he's already used to the medicine in some form.

He hasn't had a ProBNP, i'll look into it!

I've been here on the forum for a little while now, but I'm still amazed by how much useful knowledge Wendy has accumulated.

I'm really sorry about your experience with the cat. Sometimes these things happen to all of us. 🙁
