10/10 Malki General Advice Reconnecting

Hi all, Back on here as feeling a bit lost and need to recentre with all your wonderful advice.
Malki went into remission in mid 2021 then back out in March 2022. Since then she has had a number of other illnesses, operations, scans, xrays, chemo and the like. Hence her diabetes management has been a mess.

July 2022 - intestinal blockage, benign tumour growth - had surgery
Dec 2022 - Feb 2023 - UTI, multiple tests/medications, self resolved after mid 2023 surgery
June 2023 - Breast cancer - Left chain mastectomy
July 2023 - preventative right chain mastectomy
End July 2023 - single round of Chemo
Between all of those there was multiple ultrasounds, xrays, CT Scan, etc

She has an ultrasound and xray booked for 10/26 to check for new cancer however other than that I am back to being able to focus on her dosages.
I had left it in the hands of her vet for dosing increases/decreases/skip thresholds until this point, by sending him curves and the chart

Other than her BG number she has no physical signs - drinking/urination normal, good energy, strong/no nerve issues, alert, no ketones.

I am concerned with her recent numbers as the difference between morning and night figures seems huge? It seemed to be trending down for ages now they are all over the shop again.

Wow, you and Malki have been through a lot the last couple years. Is she on any additional regular medications now due to all of her issues?

The spreadsheet is working, but it's a lot to go through with multiple years on one tab. I'm tagging @Bandit's Mom who's a whiz at spreadsheets to see if she can put multiple year tabs in there.

A question on dosing. You were giving 2.25 units, then reduced to 2.0 units on 9/13. Can I ask why? Also, with SLGS, it's good to do a curve every week. You've been testing at exactly the same times during the day. Sometimes it's good to mix it up a little bit. The curve can also give you some better data on what is happening at other times.

As for differences between AMPS and PMPS, that's not unusual. Looks like Malki is going lower with better cycles at night (very common) then is doing some bouncing as a result during the day.

In regards to the drop from 2.25 down to 2 this was at the advice of our vet. His view was that with that many skips it was better to give a lower dose to make it more regular.
A footnote on Malki is that she seems to show "hypo" type signs at a slightly higher reading than standard, so anything close to 5mmol or under and she becomes glassy eyed, lethargic, and 3-4 she gets a twitchy face. So this may explain some of the reasoning to skip at lower numbers others would normally shoot. Her magic number seems to be 7-8 where she turns into a kitten and destroys the house and wants to play. I am often out of the house between each test/shoot, which makes it difficult to monitor dosing at a lower preshot number.

Yes I've fallen behind on the curves, which Ill pick up on again. I've been super restricted with work shifts, hence the clockwork timing of everything, and I have no backup person to help with her. I did have a vet nurse for a bit but she has moved on and I couldnt afford it currently anyway.

She is currently only on probiotics and cbd oil as a little break from the absolute onslaught of medicine the poor girl has been on the last 1.5years. Her spirits are high though, and she shows no signs of any pain/illness. Playful and loving, shes a treasure.
