1/2 Sid Update bg spiked to 722 last night

I went to pick up Sid and they did not let him leave because his glucose spiked to 722 last night and then they checked it every hour and it went from 722 to 613 and then down to the 300s. He had been staying pretty steady in the 200s over the last 4 days in the hospital and they check him every 4 hours so the spike came very unexpectedly. The vet there said she was going to call an internal specialist at Michigan State University Veterinary Hospital to see if they had any ideas. Of course this morning she upped his dose to 1.5 because she said she does not think he was bouncing. Do any of you have any ideas about this?

Everything else is going pretty well. His BUN was down to 54 this morning and they are finally taking him off the IV drip.

Hi guys .. how crazy his BG's jumped that high so unexpectedly .. I don't have any ideas on this myself, but I do know MSU has some wonderful vets so hopefully they can shed light on this .. extra hugs out to you guys!

That's scarey! I'm wondering if they accidentally gave a fur shot. A missed dose coupled with the stress of being at the hospital could cause a spike. I'd also ask if there was anything going on back in the area where Sid is around the time of the spike. Could he have gotten scared and they took a reading shortly after? Did anyone do anything incredibly stupid like give him another cat's steroid med? Or HC kibble?
 
Honestly, I wondered the same thing. There are a lot of whining dogs there usually, but this is Sid's 5th day and he should be used to all the noise and activity by now. I don't have complete faith in the staff anywhere. Just like in a hospital or any medical setting, people make mistakes. Happens all the time. (One of the vet techs there is a former volunteer in the rescue I volunteer for and we had to ask her to stop fostering for many reasons but one of them was not medicating the animals properly and not keeping clear records - she wasn't on last night that I know of or I would be certain that she did something.)

They actually asked me if I gave him any special treat or food during my visit last night. As if.

Even if it was a fur shot, a spike all the way to 722?
 
If it was a perfect storm of issues -- fur shot, a dog snarling in front of Sid's cage, a few pieces of contraband kibble, someone dropping a steel bowl, etc., it could happen.

As long as the numbers come down and stabilize, I'd not worry too much.

I also wonder if maybe they had some bad strips or a bad meter, although, I think those are less likely.
 
My goodness, Sid...you win a lot of awards in my book...but let's not even think about one for that high a BG!

That would have scared me, too, Lydia, especially because you have to wonder if there was some outside influence as Sienne suggests. Holy Carp. So glad to see him back down.

Also glad to see his BUN coming down but I'd be focusing more on what the creat is doing :-D

Just catching up after several days away from my computer and and read about Sid's spike. I do hope that things will settle down right away and that you'll be able to take him home. Sending lots of positive thoughts, beautiful healing vines, prayers, and a wish for a healthy New Year,

I got Sid home about 7:30 EST tonight. He's due for a shot around 8 so I tested him at 7:45 and he was 73 gave him some baby food and a small meal and retested at 8:00 and he was 45. Panicked a bit and gave him some more food and called the vet. Tested again at 8:45 and he was 157 so I have him about 1.25. His last dose of lantus at the vet this morning was 1.5. He ate another small meal and ate the whole thing. I will test him again in about an hour. He is resting right now.

last night at 8pm he was over 700 and tonight at 8 pm he is 45? Of course the vets are suggesting he change insulin. I do not know what to do.
 
Do you know what time the vet shot and if they were using a shorter acting, bolus insulin (e.g., Humulin R)?

Given where the numbers are at the moment, it does make me wonder if there was a technical problem (or something) last night.
 
Yes, the 722 was at 8 pm last night. He was given 1u of Lantus - no regular insulin. Then his next reading was 686 - that's why I question a technical problem.
I updated his SS.

They believe he has an acute kidney infection because he feels pain when they palpate his kidneys.
 
If it is, in fact, an infection, the spike in Sid's BG is entirely possible. Once the ABs kick in, numbers can drop fast and hard. You clearly got him to the vet before the infection was in full force. It also makes sense why he was throwing ketones (infection + not enough insulin + not eating --> ketones/DKA). The vets got insulin into Sid, addressed the kidney issues and his BG has come down.
