Yesterday, we did an 18 hour cycle and Ming surfed the greens very beautifully for the first half of the day. I'm also pleasantly surprised to see that he's still in the pinks this morning and not insanely high.

We're sticking with 6.5 units today because I'm not home all day and I have work tomorrow as well.

Wasn't able to get a ketone test because Ming didn't pee before I fell asleep.
 
Thoughts this morning: Maybe I should post on the IAA forum and ask for some info. I read a few articles and threads of cats who had IAA. It's, as always, overwhelming in the beginning and hard for me to understand words :rolleyes:

I can't at the moment afford the IAA test right now. That big trip I went on drained me dry :facepalm: So any "extra" expenses I've been trying to hold off until I get back on my feet in a couple of weeks. Plus Ming's bill at the vet has not been paid in full and adding onto it and working up the courage to ask a vet for the IAA test gives me anxiety :confused:

I guess my point in this long string of thoughts is: if IAA is suspected, what should I look out for in the next few cycles so Ming stays safe?
 
Almost anyone who will answer your IAA questions also posts here. There is no rush to do the test, it is just unfortunate it didn’t go at the same time as the acro test to save costs on blood draw and shipping. It’s also possible that what you are seeing is Ming getting over some glucose toxicity that developed while you were on holidays and the dose was lowered to keep him safe.

As for what to expect, it’s hard to say exactly. Especially predicting what will happen in the next few cycles. You drained the depot with two back to back 18 hour cycles, so I think higher numbers are most likely until the depot builds again. Predicting what antibodies will do is hard, as by nature they grab and release injected insulin as they go through their life cycle. In other words, it can seem pretty random. IAA usually breaks around after a year, for Neko it was nine months. At that point dose needs can drop fairly quickly, by which I mean 2-3 dose reductions a week, but not always steadily. But again not predictable as ECID.

We really don’t know a whole lot about IAA except that it is idiopathic and usually lasts somewhere around a year. When it breaks it’s hold on injected insulin it can be quite dramatic.

Whether it can come back is unknown.
I had Black Kitty retested after his IAA broke in October of 2008 and the results were the same - 84% , which is considered extreme insulin resistance. In spite of the antibodies giving up their grip on the insulin BK had in circulation, as long as he was on insulin the test results remained the same. After he went OTJ, he tested at 2%.

Hi Sandy! Long time no talk :)

Interesting and thanks for the info. I think I'll peruse BK's SS and maybe learn a few things. I read an old thread of you and BK's as well when you retested for IAA.

I have work tomorrow too but I can sleep + monitor tonight if needed.

I just don't feel safe keeping him at this dose if he drops so suddenly tomorrow since I won't be home. My day off is on Tuesday.

What are my possible options if he continues to coast around 50-99 around PS time tomorrow?
 
Yikes! Ming, time to pull up. Maybe a couple tsp food to help him surf.

If he is still hovering low tomorrow, you could skip or do a reduced dose. Too busy during the day for him to be at work?
