I think I may have figured out why her sensors keep dying early! We haven't been trimming her hair as short as it really needs to be so the fur grows out much quicker than expected and starts pushing on the sensor, causing false low readings. I have been using a bikini trimmer because I thought that was good enough but after doing some research yesterday, that only gets down to 2-4mm whereas a 40 blade trimmer gets down to 0.25mm!

So I ordered a trimmer with a 40 blade for local pickup and I think we'll probably replace her sensor sometime today.
 
We put a new Libre sensor on her this afternoon. Hopefully I was right about the fur growing out causing the other sensors to fail early. It would definitely explain why the first few didn't fail early because those were applied at the vet's office where they likely used a surgical trimmer before they applied the sensor.
