Is that common in newly diagnosed cats? to have to go up up up to finally get their body to respond? I can’t believe we are already at such a dose and she is not budging. I have enough gray hairs, Chewie, stop giving me more!

Yes, it's not uncommon. Some cats respond sooner than others. Some you have to go to a higher dose to break through to good numbers. When that happens, they sometimes come down in dosing as fast - a hairy ride for the caregiver. So fasten your seatbelt. This is a regular rollercoaster!

I assume the insulin pen/vial is just 3 weeks old and stored in the fridge?
It is, and no particles or anything. The only time it was at room temp was on the 30 min drive home but I had the AC on in the car.
I was wondering if maybe the insulin isn’t being absorbed well because she won’t let me inject her in her abdomen? I am not injecting in the scruff either, since it is poorly vascularized. I remember some members said they were injecting low behind the shoulder blade, so that’s what I have been doing.
She is peeing fine and had a dental cleaning in January, I even checked her mouth today and can’t see any inflammation.
 
I guess I am wondering how far up do we go before it’s time to go back to the vet and ask for a urine culture just in case, and/or a possible switch to levemir? I just always have this nagging worry that I am not doing something right, or something else is wrong with her that I am missing… it’s irrational but I can’t shake it every time I see those awful numbers.

She probably just needs to get to a good dose to break through. Might not be the insulin or the shooting location. Lantus starts stinging at much higher doses and that's when we recommend a switch to Levemir. We also ask members to give an insulin at least 6 months before contemplating a different one.

Some cats need more insulin than others, my girl Neko was one of those. But 4 units is a little early to worry about anything other than just not at a good dose yet. Unless of course she is reacting to the Lantus sting. We do see the occasional cat react badly to it at smaller doses. Switching to Levemir will not be a cure for the fact that she's at the point where she needs more insulin. The doses of Lantus and Levemir needed are usually fairly similar.
