Ok..at 8 last night his PM amps were 309..gave him the new pen and fed him. Rechecked at +3 and it was 293. Nothing more to eat...this morning it was 338.

I am just not getting the hang of this..it seems there is no rhyme or reason to his sugars. They should be going down some anyway???

Around +2 is when he probably has a spike from food.
Pre-shots are often the highest numbers.

The nadir, or lowest glucose level between shots, is around +5 to +7 hours post shot. That is the period and numbers to focus on to look for progress, not the pre-shots.
 
What I am saying, and yes I have been seeing those tests in history...but his sugar is constant...it is up to around 300 all the time. It does not go down much in between shots..

Okay let me see if I can explain this. Both preshot numbers should be the highest numbers he has during the day. The numbers should slowly drop until nadir (lowest point normally around +6) then slowly rise again until the next preshot. Which is why we harp on getting midcycle tests.

Lantus and Levemir are depot insulin in that they form a time released depot under the skin. Kinda like a holding tank or a funnel. Each shot builds on the one before it. Because what you shoot this morning isn't in a useable form until the next couple cycles. What you just shot goes immediately to the depot or shed what is in that shed is what he'll be using to bring down his blood sugar this morning.

Think of it like filling a funnel without putting a cork in the bottom. You are trying to keep the water level with the top of the funnel while the water runs out the bottom. If you pour too much or to fast the water can't run out the little hole at the bottom fast enough and it overflows (hypo) if you don't pour fast enough or add enough water the top level drops (high bgs) but either way what you are pouring into the top of that funnel isn't what is running out the bottom. Its what you poured in the shot before or even several shots before.

If the previous pen was bad then even though you changed pens last night he was still using the old insulin last night . It's going to take approximately 3 days for his shed to drain all the bad insulin out and fill back up with good insulin.

Every shot you give only a small portion is immediately available to KB to use to drop his blood sugar the rest is going to replace what was used the previous cycle.

Now there are insulins that are in and out. Where you shoot them and they immediately start dropping the blood sugar and are completely out of the system by the next shot just Lantus and Levemir aren't them. They would be the PZI and Novolog family. And of those only the PZI family is acceptable for use in felines as a basal insulin. The Novolog family (N and R) are in extreme cases can be used as a bolusin the hands of an experienced care giver

Does that clear things up or have I confused you more?

I am just worried that his sugars are constantly staying up around the 300...This is not healthy for him and I am worried.

I will keep testing..today I am going to try to do the every 2 hours so you all can see his blood sugars remaining high. or should I wait until 3 days to do a curve.
 
Since I know you're tight on strips lets wait until Monday to run a full curve to give the new pen a chance to be fully in effect.

Another way to determine progress is watch the 7 or 14 day average on his meter if that average is dropping he's still settling. That average is the kitty equivalent to your A1C. Some cats just take longer than others to settle.

Sometimes it also helps to look at other kitties spreadsheets to understand how the insulin is suppose to look. My two don't count their Levemir cats.

The average is 310 right now. It has been over 300 constantly, as I see it when I turn it on...I will watch it as it comes down. Ok..so I will try to do a curve on Monday..but that day I do a lot of running...football practice there and again back...Eric to work...the pick up baby from day care...Wednesdays are better as I get to skip the morning football..LOL! Just the afternoon run from 3:00 to 4:30

If Wednesday is better then run it then no biggie. Your Monday's soundlike my weekends not enough time to change my mind.
