05/23 Juno AMBG 61: Advice needed

Good morning everyone!

Juno is still doing great in the greens without almost any insulin :-D. Today his AMBG was 68 and since I'm not able to monitor him during the afternoon, I didn't shoot again in the morning. Yesterday evening I gave him only 0.25u, but I'm not sure how to proceed. Whether I should still give him a dose of 0.25u in the evening or start an OTJ trial? I got some advice from the Feline Diabetes group fb page that when Levemir is given in micro-doses, we don't need to go through the BID drop method. What do you advise?

That is great news that Juno is doing so well. :-D With Dimitri, though he was on Lantus not Levemir, I was told to take his dosage down to 0.10.
He held that dose for a couple of days, and then we did his OTJ Trial. Someone else will be along to give good advice I'm sure.

Thanks a lot! I have been looking at some of the fur babies spreadsheets who are now OTJ to learn what to do right now and one is from your Dimitri's ;-). I'll see how Juno is in the evening, but I'm hoping he keeps the good numbers :-D. I get so anxious every time I have to test him. It seems like playing the lottery :shock:.

Yup, most beans go down to the last itty bitty dose of 0.10U to give the pancreas as much support as possible before starting the OTJ trial. Juno is looking awesome!!

Hi Amy!

He's still in the greens, but I decided to follow the recommendation of most people and gave him 0.15u in the evening. The dose is so tiny that it's difficult to measure precisely if that's the exact amount. Anyway, I'll be testing him during the night.

That's so true! I'm very excited about Juno's last BG levels, but also very cautious about his numbers. I'm hoping for the best.

it's a pretty big decrease to go from 0.75 to 0.15 in 2 shots - i've got my fingers crossed that this will hold. It might - it's impossible to predict. I'm cheering for Juno to hold the great numbers, though!! Go Juno!!!!

However, if his numbers trend up, you might need to be quick to respond and go back to the 0.75 until you catch a number under 50, then reduce by 0.25u at a time. If you have to skip a shot because you can't monitor, that's fine, but usually people would go back to the same dose they'd been at.

There are pictures of the tiny doses in the New to the Group sticky, if you need a little help with it. One very small dose that is less than 0.1u is to push hard on the plunger, then insert the needle into the rubber stopper and release the plunger. The vacuum you create by pushing in the plunger will pull in just a tiny amount of insulin when you release it. That dose is probably about one drop.

Some cats need to go all the way down to that in dose. Some can go off earlier, but in general, if you can continue to give him insulin without him going below 50, you want to keep doing it as long as you can. it supports the healing, sputtering pancreas and gives him the best chance for going OTJ.

That said, Juno will do it his way, so we'll just have to try to keep up with him! Everything sure looks great. Fingers and toes crossed for our little buddy!

Thanks a lot for your advice. I really appreciate it. Juno is still doing pretty well in the greens, but as you said it's very unpredictable what will happen in the next days. Meanwhile, I'm enjoying quite a lot the reading of the past days :-D.

Perhaps I should have kept the 0.25u dose, but I thought he could go too low and I wasn't very sure about what to do, so I gave him a dose slightly less than 0.25u. I will try to keep this dose, unless he goes below 50. If his numbers go up I'll definitely increase the dose back to the 0.75u!
