When Copernicus goes to the vet school, they do NOT listen to me or treat him as an individual case. They keep saying we follow protocol. They are freakishly worried about hypo to the point that they skipped his dose all together when he was only at 99. I would have given him some insulin at 99. Being an acromegalic cat with extreme insulin resistance, I begged them not to skip his insulin. They ask for mutual trust, yet they don't do anything I ask them. He is now 529 and they refuse to give any regular insulin. They said they worry about him going hypo with a regular insulin shot. They seem to think because he went 99 last night on 23 units that insulin today with a BG of 529 would have the same result. This is not rocket science. His next shot is for 7 more hours. I told them he would go to 700 and they were dismissive. Help. What should I do?

They also refuse to feed him every 3 hours. This is why he went low!

For those of you that have been allowed to manage your cat's own insulin need while they are in the hospital, has that been a primary vet? I think this is 2 strikes that we are ready to move on from the vet school. Now remember they didn't check his BG or feed him last time and he went to 700 and caused an AKI.

How do I get him the IV fluid therapy he needs without hurting him in other ways.
 
My history is different. Way back when about 20 years ago, J.D. was admitted to the University of Veternary Medicine School of Pennsylvania with DKA. He had to stay there for over a week. I had tried to find a vet that I felt "knew something" about diabetic cats and did not find any. Upon discharge, the vet at the vet school upon learning this told me "I will be his vet from now on". I said you are 1 hour and 15 minutes drive from me during non-commute hours, can I have my regular vet to the tests you prescribe and call you and you get back to me? She agreed. So, I paid for the tests at the primary vet. and pretty much was was getting free advise from the vet at the vet school, and letting my primary vet know what tests to give. Then, one day, she moved to California, but my vets (primary) still ask me what tests I want today, and we discuss them.
I wish you had the same support.
I think my present vet has never treated an acromegaly cat ever, but trusts me to do my research and listens to me, and we talk about what we and she thinks is best.
 
I wish I had some advice for you. I'm so sorry! I can't imagine how frustrating this is. The very people you are depending on to help don't know enough to help and won't trust you, when you know best.

These vets just make no sense. I wish you had another hospital alternative that was more responsive to your needs.
They just seem negligent in so many ways with their protocols. (Maybe other hospitals are like this too, I just don’t know).
And you need the support of a vet to write you scripts as they are needed, especially fluids for at home 😭😫

We were doing fluids at home and it wasn't enough. He still needed IV support.
