I just got a call from the vet with Binx labs and theyโ€™re really badโ€ฆ his kidney values have increased 3 fold. His creatinine is 679 (apologies I donโ€™t have capacity to do the conversion right now, if there is one).

Iโ€™m just sitting in a stairwell at work completely distraught. I need to look into options and outcomes but the vet told me from their perspective thereโ€™s two:
- Hospitalize to try to bring his numbers down, an ultrasound to see what might be happening, and then aggressive treatment moving forward. They said this may only help him feel better for a short time, before needing to do it again.
- Start planning for goodbye

When Iโ€™ve calmed down, Iโ€™ll look into everything more. I will do the forms that Iโ€™ve seen shared here about knowing when itโ€™s time.

Briana, I'm so sorry to read this update. That's a pretty sudden increase in creatinine from where he was at in July. I'm still learning but from some of the reading I've done for the kidney board, if there's something acute going on like an infection or urinary issue, numbers can look really bad really fast, but can improve with treatment.

Thanks, Tim ๐Ÿซ‚ That's very good info. The first thing I started reading was this page on AKD and ACKD. It looks like Binx falls under Stage 4 of 5, for AKD and from what I'm seeing, likely days or a week of hospitalization/fluid therapy and then I read cats in later stages "may die within 5 to 10 days despite appropriate conventional management" - I would be devastated if Binx spent his last days hospitalized. And the hospitalization with his FD, hyper-t. I know they'd manage his conditions, but it makes it all so much more complicated. Just getting my thoughts out, but I appreciate the support and knowledge. I'm sure you've been in deep with the kidney board.

As Tim mentioned, hoping there is something else causing those elevated numbers that he can overcome this.
I know how hard that blow was to receive from the vet, I am so sorry you had to get that call while at work.

Oh my gosh Briana, Im so sorry. I do agree with Tim tho that it could be something acute like a kidney infection. Those are unfortunately really hard to treat and usually take many rounds of antibiotics. If all this has come on quite suddenly, you do have a chance tho with treatment that he could come back down to reasonable numbers and give you a little more time.

Any chance he was dehydrated when his blood was taken? That can also increase numbers. But I'd definitely investigate the kidney infection possibility. That does seem like quite a jump. Neko's were going up in her last year, but not that fast. My treatment of her kidneys was limited because her heart wouldn't let her have subq fluids.

Thank you, Jo and Wendy ๐Ÿซ‚ I'm going to call the vet about an infection now, because yesterday before I took him to the vet (around 5pm) he wasn't really drinking and had barely used the litter box. So also to your point Wendy, he was definitely dehydrated.

Iโ€™m so sorry Briana. I have seen cats with his level of creatinine respond well for a long time. But very concerning is his phosphorus and potassium are way too high. The potassium level is one that needs to be addressed immediately with your vet.

I hope your vet can address the labs and help his numbers improve.

Marje, thank you for flagging the potassium - as I start to read, there's of course some very useful information. Especially with potassium levels related to heart function, which will be important as we consider options/fluids.

With CKD, I find itโ€™s really important to stay on top of phosphorus, anemia, hydration, potassium, calcium. Other things can develop but alot is gained by keeping those numbers under control.

Briana, I am sending you a big hug, along with lots of wisdom and serenity to deal with everything that is needed.

If he was dehydrated, would it be possible to repeat the lab work after he is properly hydrated? This is what I recently had to do with Mami Cat, to see what changed or not after proper rehydration. ๐Ÿ™
