Otto AMPS 437 Need help :(

Hello! Otto has had a whirlwind. See last post for details. He had significant issues with high sugar. His internal med has increased dose using data from his libre sensor, he’s now on 17 units of Lantus twice daily. I recognize she was aggressive, but with his DKA and persistently high numbers we decided that was best. Of note, I switched his food last week to tiki cat aloha friends.

Today, had hypo event. Sugar was 48 and 50, he was a little wobbly but otherwise no other symptoms. Gave high carb wet food and improved to 100s. Sugar is now at 80 1 hour before shot. I’m so scared to give him full 17 units, does anyone have ideas for dosing? Emergency vet said half dose but even that scares me since he’s on so much!

We were going to test for IAA/acro at his next appointment, but haven’t done that yet. I’m wondering if perhaps he had IAA and it’s breaking? Or maybe just the food change? Any help is appreciated!!
 
OK, have read back now - thanks for the link.. What food was he eating before the switch to Tiki Cat? Any of the old dry food? Which day exactly did you switch foods? It can be helpful to put notes in the spreadsheet, specifically the Remarks column when you change things that can impact blood sugar, like food changes, medications, etc.

The above is because I'm wondering if the food change is a significant carb% change, and could mean more than a 1 unit dose reduction. I remember one cat go from 13 units to about 2 when he changed from high carb dry to low carb wet food.

Have you tested for ketones at home lately? If so, please add the test results to the spreadsheet. You should still be testing for them regularly at home.

I suspect the food change is key, it can take a couple days for the effects of removing higher carbs to show up. He could still have either IAA and/or acromegaly and at 17 units that does look suspicious. IAA doesn't break that suddenly. It could also be you are getting towards a dose that moves him.

He was eating fancy feast flaked food, tuna and trout specifically. I started to mix in new food to his old food on 12/29 and today was the first day he got only the tiki cat. I do think the tiki cat is lower carbs but the calculations I did don’t seem like it was a huge shift in carbs, i may have calculated wrong though?

Have not tested ketones recently, will add to spreadsheet once I get new strips. He did have urine ketone at vet that was negative a couple of weeks ago.

Thank you so much for your help! I ended up giving 8.5 units tonight just to be safe. Thankful I have the libre to keep an eye on the trend tonight. Hopefully we will get a better picture of what’s going on over the next few days.
 
The FF flaked tuna and trout are in the 4-5% range, both considered low carb. If that's all he was getting before hand, the switch to Tiki Cat wouldn't have made that much of a difference.
