08/19 Regs AMPS 184 +5.5 156 +8 195 PMPS 231

His pm pre is only 231 tonight, I hope that he will be fine with the dose. I just wonder why he is suddenly having so much better values? And if we should go a little lower in the future?

Beautiful blue surf today. A little bouncy tonight. He's seen a lot of bouncing, which is normal as they begin treatment.
Thank you so much Mary. He really had a great day and was not as thirsty and looked just soo happy. But after today I kind of expect a little bounce at least.

I just gave him the Lantus and just stress that he might to drop too low tonight. I thought the same this morning though

Check him at +2 to see what direction he's headed. If he's ~20% lower than the preshot number, you might expect an active cycle. If he's flat, then you might expect a normal Lantus cycle with some downward momentum. If he's ~20% higher, then he might be going into a bounce.

I'm so happy to hear that he was looking and feeling better today! As they say here, our kitties are so much more than just blood glucose numbers--we always want to focus on how they are feeling and behaving.

231 is definitely not too low of a number to shoot. Before you know it, you're going to be shooting greens! Take a look at Jude's ss. We shoot a lot of greens. That's where you want Regs to be, and eventually you will get there.

Thank you so much for that advice, need to remember that for the future! That really is a great first indicator how the next hours could be. At +2 he was at 320 so I guess poor Regs is having another bounce. He got one teaspoon of medium carb food for dinner but that should have not such a gain in sugar.

Jude’s numbers are impressive, not sure if we ever will get that low. Since we are on Lantus he definitely has a downwards trend but I really thought that he couldn’t get much better than today 😂 I guess we keep working on that

Yes, he’s bouncing from hitting those lower numbers that he’s not used to. He’s likely still got some room for an increase. If it were me, I would check tomorrow and ask about a dose increase by putting up your “?” Prefix. Early on in treatment, it’s best to get them into lower (healing) numbers, if you can. Cats can go into remission, you know, which means they don’t need insulin.

The whole night he stayed over 300, just glad the bounce did not go higher. Hopefully it is not too long this time. It’s funny I was so scared seeing the lower numbers and now I’m disappointed to see the high ones again. How fast will he adjust on the low numbers or better how long do I have to expect that bounces happen?

That would be a dream for Regs, even without diabetes, he already has enough issues.

Generally speaking, (and keeping in mind that every cat is different (ECID)), the longer a cat is on insulin and the more regulated they become (consistently remaining in lower numbers), the shorter the bounces are and the less extreme they are. Bounces are a "natural reaction to what the cat's system perceives as a BG value that is "too low." "Too low" is relative. If a cat is used to BGs in the 200's, 300's, or higher for a long time, then even a BG that drops to 150 can trigger a "bounce." Bouncing can also be triggered if the blood glucose drops too low and/or too fast. The pancreas, then the liver, release glucogon, glycogen, and counter-regulatory hormones. The end result is a dumping of "sugar" into the bloodstream to save the cat from going hypoglycemic from a perceived low. The action is often referred to as "liver panic" or "panicky liver." Usually, a bounce will clear kitty's system within 3 days (6 cycles)."

Following the dosing method guidelines and taking increases (and decreases) when indicated can help get Reg's numbers down lower. As you're beginning Reg's treatment, feel free to check here on the board regarding dose increases/decreases. If you hold doses too long--meaning when he is remaining in higher numbers beyond the dosing method's indications for dose increase--you run the risk of glucose toxicity, which makes it harder to get them down from those higher numbers. When cats remain consistently in lower numbers, they can experience healing of their pancreas, which can lead to remission. So getting him down into lower numbers will increase the chance of remission. Not every cat experiences remission, but it's a good goal early on in treatment as that is the time when the likelihood of remission is greatest (though I've seen long term diabetics go into remission).

Most everyone here understands the concerns--the fears--of lower blood glucose numbers. Right now, you are building data with those tests that are showing you how his body is responding to the insulin. That data will be of great help as you increase his insulin and his numbers go lower. For example, yesterday morning and last night, his numbers were relatively flat. He bounced last night, but look at how flat he remained. This is really fortunate for you! A lot of kitties are up and down and all over the place. Not Regs! Steady Freddy! Now, he has had some extreme spikes, like on 8/17, but generally speaking, he's fairly flat. So, I say this data helps because, as his numbers drop, you might expect the same flat cycles, only lower (maybe--cats always like to keep us on our toes!!).
