My cat Simon is still in the 400s. I just increased his dose this evening to 3.5U as suggested by @Bandit's Mom .

I have to run my neighbor to pick up her car from the shop but then after dinner, I’ll post. Since you just raised the dose, that gives us some time to discuss.

You’ve probably read under the TR protocol that if the BG is high and flat over 300, the dose can be raised 0.5u bid after six cycles.

However, for cats like Simon, spending all this time in these high numbers can lead to glucose toxicity.

For many years, we have been working with caregivers (CG) whose cats are this high and flat with “fast-tracking”. That means we have the CG increase a dose by 0.5u bid after four cycles. If the BG doesn’t drop below 300, we do it again. Still no BGs under 300, we increase by 0.5u and hold that dose six cycles to allow the depot catch up.

If we still aren’t seeing BGs under 300, we do that whole process again. When we start seeing BGs below 300, we go back to following the TR protocol as written.

You aren’t required to do it; it’s just a tool to get them into better numbers faster. You do have to be able to test so we can catch the low numbers but a PS, +2, and around nadir during the AM cycle works and PS, +2, and before bed in the PM cycle.

Okay, this is helpful. Tomorrow morning will be his 4th dose at 3.5U, so if he's still at 400 then tomorrow night I will raise it to 4U. Thank you!
 
I know it’s tough to see those higher numbers. I’m so glad you’re posting, I’ve gotten tremendous help and learned so much by following the advice I’m getting. Wishing you and Simon well and hope he starts to come down soon💕
 
Correct. We will get there a bit at a time.

How is Simon feeling?
Awesome -- will do! He sleeps a lot and has not been himself in long time. He still eats, drinks, urinates a lot more than he used to. Once in a while he will play, but mostly just wants to be close, purring and sleeping. He used to be highly active and play for hours a day, so it's been a big shift.
