? 03/21 Blanche 122@AMPS stall 30', 104@+0.30, 0.25U, 117@+1, 95@+1.30, 77@+2, 95@+3, 110@+4 To reduce or not to reduce?

Hello dear all!

Well, two days no see and it already felt two days too long! 💖

Back for revaluation as it seems that I have to reduce again tonight.
And I dare paraphrase the Bard because last time Blanche earned a reduction it seems that I did right to hold it as @Sienne and Gabby (GA) suggest it.
I think this time the reduction is well earned but I'd like your experienced eyes to confirm it (or not).

Hmm. If I'm understanding correctly, @Sienne and Gabby (GA) was cautioning against taking too many back-to-back reductions, because the depot needed time to catch up, and the higher dose from only a day or two previous could still be affecting Blanche's numbers.

It's been a few more days between dose changes, and she went below 90, which earns a reduction per SLGS. At the same time, her dose is .25u, and usually the reduction after that is a "drop," I believe. And she's still seeing numbers in the 300s, as recently as yesterday.

I'm sorry, I think I'm out of my depth! 😅 Or at least, I've reached a limit in my current understanding of how this works. I hope someone can come along soon and be more helpful. I'm glad to see that she's doing well overall, though. Hope you're having a lovely weekend.

I don’t very well know what to do. It’s sure I’m a newbie and I have to go by the book but it seems that the book holds hidden some exceptions too. So I’ll wait!

Well, the decision was made by the PMPS. An nice(#not) fuchsia 352 was not very convincing to lower the dose. So I kept the 0.25 for tonight and Blanche ate her wet RC diabetic with the customary half boiled running egg mixed with warm water. I think that it is the RC diabetic dry of her breakfast who stays in her system later than the insulin. If tomorrow she has a lower AMPS I thing I will shoot 0.15 and no dry kibbles at all.

The depot takes 4-6 cycles to adjust to a reduction and it has been 10 cycles on this dose (including tonight) so you needn't worry about it being a back-to-back reduction.

I haven't but I have print and cut very careful the paper insulin ruler for the syringes I use so I will be as accurate as possible. :)

The reduction was earned and there isn't the concern over back-to-back reductions. I'm going to make an observation you probably won't like. I would take things slowly. Blanche's spreadsheet doesn't look like one of a cat that's ready to go into remission. There's not enough green and there are still a fair amount of numbers that are outside of normal range. Just don't be disappointed if the dose needs to be raised. It's not uncommon for the doses to wobble around.

I will take any observation, even more those I don't like. And I will thank you more for them. I know she is not ready and to be honest the one thing I'm really concerned is the possibility of an hypo when I'm away. I surely don't want to rush anything. Even if I would, I wouldn't have a clue how to do it.
I just try not to let her drop below 90 and to hold the dose as much possible to let the depot build.
But what you mean "I would take things slowly"? Did you see something at her ss suggesting that I do something I shouldn't? Or that I have to do it differently?
 
You're doing great! My experience is that once new members see numbers dropping and the dose is low, there can be a rush to an "off the juice" (OTJ) trial before the cat is truly ready. Take a look at some of the spreadsheets over on the Lantus board to see what a cat's spreadsheet that loses to or on an OTJ trial looks like.

We do have a post on handling low numbers. It may be something to print out so you have it handy if needed.

I understand the temptation, but not my case. I will not do anything without posting and seeking advice beforehand.

Take a look at some of the spreadsheets over on the Lantus board to see what a cat's spreadsheet that loses to or on an OTJ trial looks like.
