New Member, BG dropped to 38, up to 56, now at 54. Need help.

Last few days my cats BG have been low in AM so don't give him his Lantus. Evening readings vary betwn 153 all the way up to 314. Vet rx 3 units 2x/day. Didn't give him a shot if he was at 153, but did at the 314. Thurs AM BG was 71, so no shot given. He vomited a bit after eating. PM test was 153, so no shot. Fri AM BG 314, gave 3 units Lantus (usual dose per vet). Evening BG 61, no shot. He hadn't eaten much, so retested at 10pm (BG46 w/only symptom being tiredness). Followed protocol of honey & wet food. Retest at 11:15pm (BG 38, only symptom tiredness). Maple syrup + wet food. Retest 11:45pm (BG 45, no chgn in symptoms, honey, churu treat), retest 12:15am (BG 50, no chgn in sympts, honey), Retest 12:45am (BG 56, no other chg, gave honey), Retest 1:45am (BG 54, gave honey). Just tested at 2:45am (BG 61, gave honey + tiny bit of wet food). Am I doing everything right? This is just so nerve-racking!

I think we're out of the woods! BG just now is 81. Will call the vet later this morning to review readings from the past 2 weeks. Will also be looking over other posts re: Lantus vs other insulin. We've been on Lantus for almost a year now and still can't seem to get his BG levels under control.
 
You've had an exhausting night I'm glad bgs are coming up now. Lantus is a depot insulin so the constant skipping doses is going to be making regulation difficult. You probably need a lower dose that you can actually shoot every 12 hours. But I'm going to tag someone more knowledgeable on lantus.

It's very numbers dependent here, if you can try and have a read through this link and set up a spreadsheet so members can help more you're likely to get better responses

I suspect your insulin dose is too high. If you can get the most recent data (2 weeks worth) in a spreadsheet, we'll be able to help you more. What I can say is that if the insulin dose is taking him below 50, you need to reduce the dose.
 
You've had an exhausting night I'm glad bgs are coming up now. Lantus is a depot insulin so the constant skipping doses is going to be making regulation difficult. You probably need a lower dose that you can actually shoot every 12 hours. But I'm going to tag someone more knowledgeable on lantus.

Thank you! I've talked to the vet this morning and they want me to stop all insulin until our appt Tues am. I'll fill out the form & take it to her as well. Will also do a curve tomorrow.

He needs less than 3 units of insulin, but probably more than zero units. If you are stopping insulin, please test daily for ketones to make sure DKA doesn't become a concern.
 
I'm not aware of this. I just tested him & his BG was 384. Decided to give him 1 unit, which should be safe. Honestly don't like the thought of stopping all his insulin for 3 days. Don't want a repeat of last night! Just too scary...
 
Sounds like a good choice, stopping completely can be dangerous. If you can get your spreadsheet set up and a few weeks filled in Wendy will be able to give you really good advice and hopefully in a bit of time get Kuro regulated 🤞
 
Seeing all of your test results in a spreadsheet gives us a much better sense of how the insulin is working along with what to do regarding dose. I agree with Wendy -- it's likely that the dose is too high but not giving any insulin when you're seeing numbers that are outside of normal range suggests your kitty needs some amount of insulin. Nice job of managing the numbers!

Thanks for the support! Doing a curve today, sticking to 1 to 1.5 units and although his number are still on the higher side, they are gradually coming down throughout the day (started at 331 at 8am and at 3:15 down to 235). So although 3 units worked 2 months ago definitely seems like its too high now. Will fill out the table tonight.
