11/30 Bingo - AMPS 227, +1 143, +6 126, +10.5 80 - Shot after post exercise crash? (Libre, Lantus)

So sorry to post for help two days in a row. ☹️ Shot time is in about ninety minutes. With his history of DKA I am thinking a token dose?

Background: Until a few hours ago Bingo was having a good day and I was so excited to give his PM shot without worry.

Then he got VERY playful for a bit, chasing toys, zoomies, the whole deal. Almost immediately afterward he seemed nauseous, threw up a little water, and started pinging low glucose alarms on his Libre.

I had no idea exercise could do this! I've been feeding intermittently for the last 2.5 hours and now AMS +10.5 he is mostly swinging in the 70-to-low-80s range on his Libre while he sleeps, but occasionally dipping low for a minute (which is scary but could be the sensor!) and getting more food.

I know I am not supposed to skip a dose. But if he is still under 100 at shot time, give .5 maybe?

About an hour to shot time, right? It looks like he is going up at this point. I would test 30 minutes prior to shot time, and repost to indicate where he is. Also, I would remove the "911" prefix and replace it with the "?" prefix since this is not an emergency but rather a request for guidance in shooting. You can indicate in the title of the post that you are waiting for dosing guidance. Also, can you respond back with the carb value of food you've fed and when you fed it as that is certainly going to influence the preshot numbers.

Hi Mary! About 30 minutes to shot now. Thank you, I changed the header.

We fed his Royal Canin Glycomax that the vet prescribed, which I've been told here is HC. (I have read the forum links about transitioning off this-- started trying today.)

I guess I'm worried because he goes up and then still dips back down 20 min later. It does not feel like he is going up so much as we are keeping him afloat with feedings.

UPDATE: So sorry I missed your request for the test. On the Libre he is roughly level for the last hour. Will test on our Alphatrak we have as backup in a few minutes.

The last test on the spreadsheet is +9. Is there a more current one?
Yes, sorry! Updated. We use Libre so we have constant readings and they sort of backwards smooth themselves.

Keep in mind he was receiving small amounts of food (fixed these hour markings, I had them off by one):
At 10 he was 84
At 10.5 he was 74
At 11 he was 99
He has not received food in 1h20 and it is looking like his preshot will be in the high 70s.

There were dips into the 50s-60s between some of those (although it is hard to tell with the Libre when those dips are overexpressed).
 
Kate, I cannot give dosing advice, but I've tagged a few people for you, and I hope that they will weigh in soon. I know that with DKA, they suggest that you not skip a dose, so please keep that in mind if you do not hear from someone before your shot time. Also, since you have been feeding high carb food, please recognize that that food will remain in Bingo's system, boosting his bg for about two hours after ingestion, after which you could see a drop in bg. You'll want to be careful to monitor to ensure his bg doesn't drop when the hc food has left his system.

I haven't kept up with the latest versions of the Libre but in the past, I've seen some pretty wonky readings in the Libre. Some random dives later in the cycle for a few mins etc. We suggest cross-checking any reading that seem off with a meter.

How long now till the shot? When did he last eat and what?

Yes, that’s doable. Stupid but… he isn’t in hypo, right? With the dips between constant feeding?
No. I think he didn't drop below the normal range (which is below 68 with a pet meter, 50 with a human meter) at any point, right? In any case, we use the term "hypo" only for a symptomatic episode of low numbers. Low numbers, while not something to be casual about, are not a concern when you are monitoring closely and feeding. While we don't want a cat to be in below normal numbers when on insulin, that does happen.
 
We didn’t discuss dosing— they were just saying it was safe to give insulin. I did just give 0.5.

Sounds like the doctor will call tomorrow (and hopefully calm me down in the process).

Sorry, when I say we didn’t discuss dosing, I just meant that wasn’t my question to them. (Didn’t mean to sound so blunt!) Maybe it should have been. It was through the front desk.

Husband is helping to keep tabs on Bingo tonight.
 
Back awake now! Libre has been doing wild stuff, recalibrating over and over.

We tested him on the Alphatrak at +5 and it was 127. 😥

Since the Libre lows are so unreliable I am wondering if we hold .5 tomorrow or if .75 was actually the better dose.
