Hello!

Jake was having amazing stable, flat curves for quite a few weeks now but has suddenly started having big spikes again, with only small periods of time at ideal nadir.

His diet hasn’t changed. I opened a new insulin vial to assess if perhaps the old one was less effective now, but it has made no difference.

He is symptomatic with excessive thirst again like when he was first diagnosed.

Does anyone have any ideas why this would be happening and/or what I should do to fix it?

I see Jake has been to the vet since your last post here: 28/10 Jake dose advice. Did the vet by any chance take a look at his mouth and see what shape his teeth are in? Worsening CKD can also make them thirstier.

Jake's nadirs are still pretty good, in spite of the spikes. Any change in food or feeding times? How late in the cycle does he get food?

He hasn’t been to the vet since 11th Sept but they said his teeth looked good then and he had a dental about 18 months ago. It could be worsening CKD as his levels were slightly worse then than they had been prior.

He has been getting food a bit earlier actually so maybe that’s why. I’ve been feeding him at AMPS and then at about +9 before PMPS due to work the last few days. I usually just feed at AMPS and PMPS. Though, his worst levels appear to be in the mornings before his insulin. It hasn’t come down much today either.
 
If you feed carbs after nadir, that can shorten the duration of the insulin. Meaning it's effect wears off sooner and his numbers will zoom up to the next preshot. Several smaller meals earlier in the cycle are find. Easier for CKD as well as diabetes. If you aren't around, you could try an autofeeder.

He did hit 2.8/50 yesterday, so almost a reduction. He could be bouncing too.
 
If these numbers continue overnight, should I take him to the vet in the morning? I don’t understand how we are back at HI after weeks and weeks of blues and greens. I don’t think he is bouncing as he has had way lower numbers and never done this.
 
Do you still have the previous vial of insulin you could try in the morning? Just trying to eliminate a bad vial as a reason for the numbers. The numbers seem to have gotten much worse since the new one.

The vet could check to see if there is a reason for the increasing numbers. Maybe a UTI? Common enough for CKD and diabetes.

Good idea. I’ve tried the old vial this morning. I can’t get a vet appointment until tomorrow afternoon, hopefully I can get the levels down before then. He is still vigorous and eating/drinking so that’s reassuring.
