Oregano. 3/10 amps 228, +1 198, +2 284, +3 271,

Hi all,

so oregano has been on 2u bid for 2.5 days now (began that dose the evening of the 8th) and as you can see in her spreadsheet today she spent a goodly portion of it reading “hi” which on the libre 2 means 400 or more.
Do I keep her at the 2u bid for another 4.5 days to do a full 7 days? I don’t want to make the mistake of dose hopping again but also don’t want to be giving 2 when she may need more. I have read through the stickies and other messages and it seems like the prevailing advice is to stick with this level for the full 7 days then adjust based on the data but just wanted to make sure. I really long for those days of blue readings like we had a couple weeks ago. I screwed that up when I changed her dose in response to her eating and am kicking myself for it now.

In other and better news it seems like what we thought was a uti may not have been as her urine has been clear for the past 2 days with no sign of clots or blood, and her appetite seems to have returned.
Also her ketostix tests are negative each day when I test so that’s good news.

Now if I can only get this dose right.

Normally, with SLGS, yes you would hold the dose a week. However, there is some latitude and because she “might” have an infection (you never said if you had a C&S or how those UTIs are diagnosed), and she’s pretty high, I would increase her dose to 2.25u twice a day tomorrow morning unless she surprises you with a BG below 90.

I actually think her best dose is going to be at least 2.5u but possibly more.

Note to readers: this is an exception because of Oregano’s circumstances. Please do not follow this custom dosing.

ok. Sounds good and will do. Up to 2.25u twice a day starting tomorrow am.

normally when we suspect a uti we take her in for a culture. She also has stress related inflammation in her bladder related to the paralysis so she can often present like it’s a uti when it isn’t. So we generally wait a couple days before putting her thorough the culture in case it wasn’t a uti and it goes away on its own. Which is has seemed to this time.

Though tonight around +1 she came down to 370 then 359 then 339 in about thirty minutes total so perhaps it is finally starting to take. I wish I understood the depot better. I understand what it means but can’t make heads or tails as to why her numbers vary as they do. Regulations seems elusive.
I know I am supposed to wait five more days at this dose before increasing so will, but it seems a long time to stay static at a dose that seems too low.

I know it’s hard to see higher numbers. I’m glad you caught some BGs below 200 yesterday. She headed up pretty quickly though which indicates a loss of duration. That can be if you fed her either more food or a higher carb food than she normally gets or we still have a bit to go in dosing.

However, when we see BGs below 200, we like to hold the dose. It looks to me that on top of loss of duration, she’s also bouncing from the lower numbers. That is a normal reaction from the liver.

I know this is frustrating…been there. But we have to have a little patience to get her depot sorted out with the consistent dosing. Then we can see what she might actually need for a good dose and slowly move towards it. Glad you are continuing to check ketones

Ok. Will hold the course. I am updating her spreadsheet here shortly and things did get better last night. Lots of mid to high 200’s. We will stay at 2.25 for the full 7 days and evaluate then.

hello, I am hoping to get some dose advice here. Today is day seven at 2.25u morning and night, and I am thinking she may need to be moved up a little. Does 2.5u twice daily seem appropriate at this point (starting tomorrow)?
