I'm a bit confused as to when I'm supposed to increase his dose again with TR. The Tilly Diabetes site says to hold each dose for 5-7 days, or alternatively, to increase every 2-3 days if his nadirs are always above 200. Our page says to increase after 3 days if nadirs are higher than 200 but less than 300.

Charlie is mostly above 200, sometimes just under, and sometimes over. lol. I held his last increase for 5 days (mostly because I hadn't chosen a dosing method yet).

So my question is: when should I increase his dose?

Any help is appreciated!
 
FWIW, I'd go by what is in our Dosing Methods Sticky. There are mostly the same, but some minor tweaks.

There's a bit of waffle room when the nadirs are under 200, which his are. TR says you can hold 6-10 cycles. I tended to hold for shorter for higher blues, longer for low blues. This is where you need to get an idea about how Charlie works the Lantus, which you will do over time. Some people find they need to hold longer for low/medium blues, some don't. With all the over 300 you are still seeing, I suspect he's not quite at a fitting dose yet. But let's see if he shows you anything better tonight, before deciding.

Okay, thanks for explaining. That sounds good. I’ll try to wake up and get a +6 tonight too. He hit another 400 earlier (ughh frustrating) but then dipped back to high 300s. I don’t know what the heck is going on, haha
