There is always a toilet, but also go out a little in the yard.
She did it other times in the past but since he was diagnosed with diabetes he does it more often. One reason now is that he doesn't go out and doesn't have food like before(?).Maybe..

Sometimes cats pee in the wrong place when they have a urinary tract infection, which is common in diabetic cats. They do this because it hurts to pee so they assume the problem is the box and try peeing somewhere else to avoid the pain
