? Ketones/increase dose 04/05 Puddin AMPS 266, +2 432, +8 296, PMPS 255

It has been 4 days (8 cycles) at 2u. Nadirs in the 200s...no real changes overall in BG; do I increase .25u?

Also, I tested for Ketones and came up with "trace" reading. What's to be done about this?

Additionally, his Flovent treatment doesn't seem to be changing anything either; he still has noisy breathing on the inhale.

Honestly nothing seems to be working for Puddin and even the vet is stumped. Vet suggested going to specialist. I don't understand how Puddin has had no changes in readings from 2.5u to 9.5u on Prozinc and still same results on Lantus. And he's actually drinking and peeing more than before insulin if I'm remembering correctly.

I'm confused!

I think part of the reason is that you started lantus too low. In answer to your dosing question, yes, increase another .25. You actually can increase every 3 days until you see bg under 200.

I hope Puddin starts to feel better and see better BG with an increase. :bighug::cat:
 
For trace readings on ketones, you want to make sure he gets plenty of liquid. Add it to his food if you aren't doing that already. You want to try to flush the ketones out. Given Puddin's trace ketones, and that he was on a higher dose of Prozinc, I'd be tempted to increase by 0.5 units. More insulin also helps ketones.

Did you vet ever mention the word "stridor" when describing Puddin's breathing sounds? You can google/youtube examples to see if that's what it is. Stridor can be caused by narrowing of the pharynx, or extra soft tissue growth there.

Since he started insulin, how has his weight changed, if any? I have a theory, but would like some answers first.

I can't believe I didn't think to add extra water to his food; that's a great idea. The vet actually also suggested adding 1/64th tsp salt to his food. I went ahead and increased to 2.5u.

Here's an overview of Puddin's situation that might give you some extra insight:

I took Puddin to the vet on 9/5/23 because he had a cough and had started to pee and poop outside of the litter box. He was 22.25 lbs...he's dense naturally, not obese. He was given a shot of depomedrol. I brought him back a few days later because his appetite had decreased but was drinking a lot - they said probably due to depo shot. A few weeks later we started to notice he was losing weight which i thought was water weight from fluid retention as this was the case for another cat of mine who has been given the depo shot before...by the time we brought him back to the vet in December he was 14.5 lbs.

Puddin is sitting around 16 lbs currently. He has a large appetite.

His breathing sound occurs on the inhale and it does sound like stridor...we've always said it sounds like a snore and that we can hear him from across the house.

Did you have him tested for acromegaly? This is only in my head because my guy was recently diagnosed with it. @Wendy&Neko do acro cats lose weight though? Shooter has mostly maintained but never significantly lost like it sounds here.

He has not been tested. I mentioned this a few times to our vet and he doesn't feel the need for that test since (I think) he's not seeing the signs of it.
