When I asked about acro signs, I was talking about more than just weight. Are you familiar with the clinical signs of acromegaly that cats can show?

Yes, clinical signs including thirst and peeing (polydypsia and polyuria) but these are sporadic. Of course, DM but that's the most obvious. With the exception of significant weight gain (which may also be a function of his lack of activity), he does not show any real enlargment of extremities, not even in his jaw, chin or skull. He is still walking properly, although I can see he has some arthritis. Having said that I've also seen him walking a little stiffly but this might be weight as well as arthritis. In his last vet visits (Jan 2024) (including emergency), his heart was pronounced sound (i.e. no murmur detected) and there's been nothing said about organomegaly. His creatinine, urea, all of that good in the last (January 2024) lab report he had. The vet yesterday even commented on how good the report looks. I don't see him acting as if he has any neuropathy or visual impairment.
Blue had a bit of the stridor breathing and some stiffness but other than that he did not have any other clinical signs, even when we went to Vancouver for the CT scan before we went to Calgary for the SRT where he sadly went into cardiac arrest. I understand that in "late stages" the issue is cardiomegaly, systolic murmurs, and congestive heart failure.
It's just this demonic insulin resistance. We're now in the 3rd iteration of the additional Cabergoline so, well, what can I say....All I can do is keep on it, it seems. I'm of course worried about getting dental but that won't happen unless he's clear in echocardiogram/radiograph . We are having a thorough pre-exam on the 16th of March.
 
@Wendy&Neko was there something that you wanted to point out re: clinical signs of acromegaly??

Anyway, here we are PMPS 308, which somehow, absurdly, seems a relief.

A cat doesn’t officially have acromegaly without symptoms. They just have a pituitary tumour, or hypersomatotropism. He does have diabetes though, and that is a symptom. I have seen it mentioned as a late stage symptom too.

I didn’t want to point out anything regarding acro, just wondered if he had any other signs of it other than insulin dose. If not, that is a good thing.
 
@Wendy&Neko just tested Eddie PM +8. Besides still being high he wouldn't take the little treat he usually gets for testing and for taking his medication. If he is still doing this at breakfast I don't know what I'm going to do. He takes thyroxine, DDAVP, Prednisolone and Gabapentin and currently Clavaseptin. He took the little Charu treat with his Cabergoline but I'm really worried we're in a spiral in which he won't take his medication because he won't eat and that means for insulin as well.

Also given these numbers at what point need I increase?
