Hi again! We started Lantus at last!! My kitty is calm and she doesn’t bother the injections! We started on 16/2 at 22:00 first injection 1 U! PMPS 296, +6 279, AMPS 266, +4 221, +6 202! I think it goes well, no? I haven’t found nadir yet! I check every two hours for the first days! Thank you!

Lantus is a depot style insulin, meaning it can take several days at a dose for the depot to build and see what the dose can do. You should see the numbers improve over the next few days.

It is really hard to figure out the nadir in the beginning, but once you start to see more variation in the numbers and seeing lower numbers, it will become easier to see it. Nadirs also can change from cycle to cycle.

If you haven't already, take a read of the yellow starred Sticky Notes at the top of this forum. At some point you'll have to decide which dosing method you want to follow too. The descriptions of the two dosing methods we use are in one of those Sticky Notes.

Welcome Maria. I had to take a double take at your kitty's name. My cat is Karamelle! Good luck with your journey. My kitty was diagnosed at the end of Dec. so Im quite new as well. Im still learning and still dont quite understand how the insulin works or how to get it regulated. Hopefully you will catch on easier than I have. Good luck.

I am adding the second cycle it looks that is falls! Should I be afraid of going too low? I will test again in an hour!

Btw, when did you switch her to Purina Dm canned? Feb 9th? Is that what she is eating now?

Have you been able to think about which dosing method you want to follow? SLGS or TR?
Hi!! Yes exactly 9th evening the first meal of purina! Yes that is what she eats! We are going the slow one right now! I will be home until Tuesday and then I am going back to work. So I will be out of house for hours from Wednesday. If there is a problem off course I will stay longer

OK. With SLGS she has earned a reduction in dose by dropping below 90. From tomorrow you would go down to 0.75U.

Do you have syringes with 1/2 unit markings? This is the one that is available in UK and EU:

She has stopped dropping. Good. Did she get food at +4 and +6? When you get cycles like last night when numbers are dropping, feed a little food after each test to slow her down.

Lantus dosing is based on nadirs. How low a dose takes the cat. Newly diagnosed cats will bounce from the lows because their bodies are no longer used to lower numbers. As they get used to lower numbers, the bouncing will reduce and eventually stop (in many cats).

What does your schedule look like for a typical week? We don't alternate dosing methods, but you can stick with one or the other for a while before switching. Maybe we can help you figure out what would work for you.

I see! I leave home 11 o clock in the morning and I am back at 21:30 more or less! Except on Thursday that I am back at 22:30. Saturday I leave home only 11-14:00! Sunday all day home
 
So looks like you are gone out of the house 10.5 hours on a typical day, 11.5 on Thursday. I was hoping there was a way you could shoot insulin and get a +2 test before you left home. If the +2 was quite a bit lower than the preshot value, it'll be a more active cycle needing more monitoring, or you would have to leave more food out for him to keep himself safe. However, if you are getting home at 22:30, the earliest you can shoot in the PM is shortly after that, say 22:45. Meaning the morning shot is around 10:45, just before you leave.

With TR, you have to get 3 days in a row with two tests each cycle, and it's just not possible with your schedule, which I think means you have to follow SLGS for dosing, to keep him safe. So down to 0.75 units for the dose in the morning.

Maria can take a few days off work now that she started insulin. Do you think she could start with TR for a few days and then switch to SLGS when she returns to work? Would that help at all?

Yes, she can do that and hold the dose for now. Considering how she is responding to insulin, it's possible she might earn a reduction with TR also! Although we've had cats who show a great first reaction to insulin or to a dose, only for those good numbers to disappear and for the cat to need more insulin.
 
I'm a bit worried that Karamela saw a 75 only three cycles into starting Lantus. And that would be without the depot completely built yet. If you lower to 0.75 units, you can make sure that's a safe dose before you start back to work. You could stay with 1.0 for one more day, but I wouldn't stay with 1.0 units until you go back to work.
 
Thank you for your responses I will have a look to the amps and I will do TR just for today to see how it goes!
