Can someone help Max? Insulin Resistance

Hi admin!
17 yrs old Max ( 3.92kg) had DKA on 12/1 and discharged from the hospital yesterday.

If a cat does have insulin resistance, then what do you do? I could not find a lot of literature on this topic for felines in Chinese or English.

He had 2 unit of Lantus last night at 2200LT and 3 unit at 1000LT in this morning that approved by the vet. However it’s still HI now (1659LT).

The vet said he may have DKA again in 3-5 days and he reminded me that due to his age, lots of health issues ( serious bone spurs, bad gum issues, kidney problems cause of inflammation and insulin resistance) he won’t be able to survive in this situation.
The vet not recommended to remove his teeth as it takes an hour for the surgery he can’t bear it.

Can someone give me some advice for this situation?

Thank you for your help, I feel stuck trying to sort through this information, I will stay at home to take care of him. I just want him to have a quality life in his last days, minimize pain, and let him die peacefully. heart breaking

Thanks a lot. Good news is Max's BG level has dropped to 15.7 mmol/L (282.6) at 0516am HK time(Now is 0553am) after I gave him 4 unit of insulin at 2100 LT ( an hour earlier than the normal injection time).
May I know where I should post my question?

You've posted in the right place. The people that can answer you have been tagged, they just arent available right now. You could change the title to include a question mark. I don't see any numbers for 2024 either. Do you have somewhere else you are tracking this info? Or has your cat been hospitalized from 12/1 until 1/16 like it states above? There is information until 12/30. I don't see a 2024 ss.

I didn't fill in the spreadsheet in 2024 hence i had installed the Freestyle Lirbre sensor. I can upload the Freestyle LibreLink report.
Max was staying in hospital on 12-15/Jan.
 
Everyone that uses a libre still fills out a spreadsheet. It is fine if you dont have the numbers from when he was in the hospital. Just fill it out to the best of your ability. You can also indicate the dates he was hospitalized on the spreadsheet. Having a ss silled out is the only way for someone that is helping you can see what it happening with your cat.

I'm glad kitty is home now.

Thank you for the prompt reply, think I have to PM the admin to get the new SS as there is no template button at the bottom of my old SS so I couldn't find the new 2024 SS. thank you.

Unfortunately, I have never worked with a world ss. I'm not sure if it is done differently. I'll tag you in the post about creating 2024 ss only if it discusses the world ss. If you know how to do it, disregard the tag.

Hi there when posting we don't use times since we are all in different times , where you said Max's BG level has dropped to 15.7 (282) at 0516am Hong Kong time that doesn't help if a member is helping you
You need to use for example @+3, or @+4 whatever it was
Also we don't know what this means when you put
at 2200LT , 1000LT or this (1659LT).

I'm confused how long Max was in the hospital ,can you tell me the date and year he was admitted and the date and year he was discharged

Can you go to your spreadsheet and in the remarks section type( in hospital ) and go to the date he was discharged and put that in the remarks section

We don't decide to shoot based on preshot values, but rather how low the dose is taking the kitty. It is possible the higher dose you are giving him is causing him to bounce into higher numbers. Here's a description of bouncing:
Bouncing is simply a natural reaction to what the cat's system perceives as a BG value that is "too low". "Too low" is relative. If a cat is used to BGs in the 200's, 300's, or higher for a long time, then even a BG that drops to 150 can trigger a "bounce". Bouncing can also be triggered if the blood glucose drops too low and/or too fast.The pancreas, then the liver, release glucogon, glycogen and counter-regulatory hormones. The end result is a dumping of "sugar" into the bloodstream to save the cat from going hypoglycemic from a perceived low. The action is often referred to as "liver panic" or "panicky liver". *Usually*, a bounce will clear kitty's system within 3 days (6 cycles).

Switching to Lantus suddenly if they were using R means it would take time to see how the Lantus was working. It can take 5-7 days to see how the dose is working. Also, we don't do increases in whole units like they have been telling you.

I'm sure those are 24 hr times.....Like 2200 is 10pm, 1000 is 10am, 1659 is 4:59pm.

I'm also sure when she says from 12/1 to yesterday, she means from January 12-January 15

When we talk about "insulin resistance," we are referring to a specific condition whereby the cat is producing insulin autoantibodies. There is a way to test for this condition. Basically, what it means is that your cat's body is reacting to insulin almost like he's "allergic" to the insulin. However, it is a self-limiting condition and you need to be aggressive about dosing in order to get ahead of the antibody production and get your cat into better blood glucose numbers.

Your vet may be referring to something other than what is clinically considered insulin resistance.
