Happy Sunday, everyone -- and since this is my first post of 2024, Happy New Year to all! It's been a busy stretch, between winding up a project at work and the holidays and making time to hang out with my sister during her visit and now taking care of a friend who's recovering from back surgery (good thing my diabetic cat has given me a solid background in nursing :p).

The good news is that Chispa's doing great on her chicken-free diet, and the switch to SLGS seems to agree with both of us too. It's kind of cool not to stress about numbers so much, and I really appreciate not having to stay up half the night testing and worrying that she's going too low. She's hit the SLGS reduction point of 90 enough times that we've gone from 2 units on 12/24 to 1 unit now -- I think the diet change must be the reason for that.

Chispa and I send love and wishes of good health to all sugar kitties and their devoted servants. And I resolve not to disappear for weeks on end in the new year! :bighug::cat::bighug:

I’m glad she’s doing better and you’ve figured out a diet that works.

When doing SLGS, you should hold the dose a full seven days before making a dose change. Usually, on the 7th day, you’d do a curve but because you have enough data during the week, a curve is not necessary. It would have been better to hold her dose two more days before reducing and the last dose was reduced a little too soon as well especially considering that you weren’t able to test as much (holidays, I get it). Certainly a curve would have been required before you did that reduction since you had no daytime data.

Remember that we don’t want to reduce the dose until we know how low the current dose is taking her. At this point, I’d let the 1u ride but please give it a full 7 days and if you aren’t able to keep up the testing as you are doing, she will benefit from a curve on that 7th day before you decide on the next dose.
As we move towards regulation and, hopefully, remission, we want either to be strong and the way they are strong is following the method of regulation as written until you get enough data under your belt to warrant custom dosing. Since you just started SLGS, I don’t think it’s quite time to start custom dosing.

Hi Marje — thanks so much for your input on our switch to SLGS. Looking back at the SLGS sticky, it does say to “hold the dose for at least a week, Unless your kitty falls below 90 mg/dL (5 mmol/L). If kitty falls below 90 mg/dL (5 mmol/L) decrease the dose by 0.25 unit immediately.” I admit, I found it surprising that it would be ok to take reductions so close together, but I went back to review the sticky each time and it seemed pretty clear that that was what I should do. Is this unwarranted confusion on my part, or should the sticky should be reworded?

My apologies. I have no idea where my head was when I was looking at her SS but you are 100% correct. I’m so used to looking at TR numbers that I let those below 90s slip by me. Thanks for bringing that to my attention. You did great!
