Thank you!! I appreciate it. I gave him the food around 2AM and after that his numbers started going up a little bit but never went too high. We're starting at a 401 this morning and based on last night I'm gonna move forward with the 1u - would like to see him nice and stable and semi-predictable in the 200s and 300s because the way his BG shoots up and down in 30-60 minutes sometimes, aiming for 100s feels too risky right now. About to chat with his vet too now that she's back in-office to discuss budesonide vs Proviable - the Proviable has maltodextrin and several sugars in it so I don't love it but it's what the ER vets recommended in place of budesonide.
I just want to encourage you to be braver about the dose.
You are comfortable with 1u but it is not the best dose for him even if he did not have DKA. When the BG sits in the 200s and 300s, it is above “renal threshold” so glucose is constantly released into the urine. This is not good for his body and overall health but it’s also not good for his kidneys.
Additionally, he will experience
glucose toxicity. Even if you scroll through that post, you’ll see this is not something you want for him. And….the elephant in the room…..a cat who has had DKA will be prone to another occurrence. He’s not entirely out of the woods right now and that’s why you need to get him into much better BGs.
Fluctuations as you are concerned about are normal especially when starting/restarting insulin. Some cats never flatten out. Look at my Gracie’s SS. Look at any member’s cat’s SS.
You’ve spent alot of money getting him to this point and I have to strongly encourage you to take the next step which is giving him the amount of insulin his body needs and will prevent him from lapsing back into DKA.
