Every meter has a 20% variance so he’s pretty flat.

I checked 15 minutes ago and he was at 299 and now he's at 233 - looks downward and I'm definitely freaking out a little since he's less than 3 hours out from his dose??
This is really nothing to worry about. A normal curve will come down and this is pretty gentle.

Thank you for responding. Is it okay even though it's so soon in his cycle and there was never any type of increase after he ate? I feel like it's just going to keep going down - his lowest numbers tend to be between +8 and +11.
I don’t see that on his SS.
I think it’s fine for you to feed him a little more and go to bed.

My sister and I are going to switch on and off checking him periodically because I do want to have a FULL curve at least a few times but I gave him some more FF and it's her "shift" so I'm going to sleep for a little while.
When you've been moving around doses like you have, it's not worth doing a curve. Lantus is a depot style insulin, and needs several days of consistent dosing to build the depot, then you can do a curve. Time for both you and your sister to go to sleep.

Thank you!! I appreciate it. I gave him the food around 2AM and after that his numbers started going up a little bit but never went too high. We're starting at a 401 this morning and based on last night I'm gonna move forward with the 1u - would like to see him nice and stable and semi-predictable in the 200s and 300s because the way his BG shoots up and down in 30-60 minutes sometimes, aiming for 100s feels too risky right now. About to chat with his vet too now that she's back in-office to discuss budesonide vs Proviable - the Proviable has maltodextrin and several sugars in it so I don't love it but it's what the ER vets recommended in place of budesonide.
I just want to encourage you to be braver about the dose. You are comfortable with 1u but it is not the best dose for him even if he did not have DKA. When the BG sits in the 200s and 300s, it is above “renal threshold” so glucose is constantly released into the urine. This is not good for his body and overall health but it’s also not good for his kidneys.
Additionally, he will experience glucose toxicity. Even if you scroll through that post, you’ll see this is not something you want for him. And….the elephant in the room…..a cat who has had DKA will be prone to another occurrence. He’s not entirely out of the woods right now and that’s why you need to get him into much better BGs.
Fluctuations as you are concerned about are normal especially when starting/restarting insulin. Some cats never flatten out. Look at my Gracie’s SS. Look at any member’s cat’s SS.
You’ve spent alot of money getting him to this point and I have to strongly encourage you to take the next step which is giving him the amount of insulin his body needs and will prevent him from lapsing back into DKA.

Hi there. I do hope you will continue testing for ketones. It’s very important. I thought you were purchasing a blood ketone meter— because I mentioned that I had bought a Nova Max Plus blood ketone meter from ADW Diabetes Supply. I could be mixing you up with someone else?
As to his dose, he had a gorgeous cycle on the 2 units on December 17. All of the stalling in the evening cycle of that day and the reduced dose only sent him back into really unhealthy numbers. I know that these lower, more normal, healthy BG numbers are new to you, and I understand that you find them scary, but the higher numbers that he is seeing are very hard on his body, particularly his kidneys. It is apparent that 1 unit is not a sufficient dose for him at this time. Since he has already been in the ER, it is imperative that you get his numbers down. We look at a lot of spreadsheets here, and when I look at his spreadsheet, I don’t see anything alarming about his cycles when he was on 2 units. What I do find alarming is the blacks, reds, and pinks of the last couple days.

That blood glucose range is too high for a cat with recent DKA. You want his numbers lower to prevent ketones. Please try to test the ketones at least twice a day when his numbers are this high. Don't rely on a lab test to tell you that you are all clear. Ketones can go from trace to large very quickly.
Glad to see you are restarting the budesonide. Irritation in the bowels can also contribute to anaemia.

Hi Rebecca! I just wanted to let you know that I have been thinking about you and Yahiko a lot this afternoon. You have been through a very scary, confusing, and emotionally taxing situation since Yahiko went into the hospital. I hope you are holding up okay. We all care and want to help. Of course, we can tell how much you love him!
