Help with dosing now that I’m home resting

Hey everyone, me again with questions lol.

I started home testing multiple times per day (with the exception of yesterday as I was out all day and only managed to do AMPS and PMPS). I’m working on doing a curve today. For the first time ever Mae has been in the 200s all day! She’s eating more wet than dry now and basically ignoring the dry food (Purina DM) so hopefully her stomach isn’t too upset.

anyway now that I’m getting data and transitioning her to low carb food, how do I know when it’s okay to either increase or decrease her dose of 2 units? She’s been on the 2 units for over a month but I am only recently home testing and adding in the FF wet. If she’s still in the 200s at PMPS, would I give 2 units? She’s due around 7:45 tonight so I’m sure I can test her at least once after that.

I’ll be home pretty consistently over the next 5-6 days so really hoping to get her a little more regulated.

It depends on what dosing method you choose when an increase or decrease is needed, either SLGS or TR
Since Mae is still eating some dry food you will need to follow SLGS

As long as you are feeding any dry you need to follow SLGS. I’m glad you are now testing. That’s a long time to be in the same dose without seeing blue or green. I think you can safely increase to 2.25. You will hold 2.25 for 7 days unless the bg drops under 90. If that happens you reduce at the next shot time back to 2 units for another 7 days unless another reduction is earned with another bg under 90.
 
From the sticky on dosing methods:

  • If nadirs are more than 150 mg/dl (8.3 mmol/L), increase the dose by 0.25 unit
  • If nadirs are between 90 (5 mmol/L) and 149 mg/dl (8.2 mmol/L), maintain the same dose
  • If nadirs are below 90 mg/dl (5mmol/L), decrease the dose by 0.25 unit
As your cat's blood glucose begins to fall mostly in the desired range [lowest point of the curve approaching 100 mg/dl (5.5 mmol/L) and pre-shot value around or below 300 mg/dl (16.6 mmol/L)], do lengthen the waiting time between dose increases. If you decide to change another factor (e.g., diet or other medications), don't increase the insulin dose until the other change is complete (but decrease the dose if your cat's glucose numbers consistently fall below 90 mg/dl (5.0 mmol/L) as a result of the change).

Don't be tempted to rush the process along by increasing the dose more quickly or in larger increments-- no matter how high your cat's blood glucose is! Rushing towards regulation will cost you time in the long run, because you may shoot past the right dose.

So I just started the transition from dry DM to FF pate. I asked for some samples of Dr Elseys and Young Again and waiting for those to come in so I have a dry option on hand if needed as my other (non diabetic) cat isn’t as crazy about the wet food. Since I’m transitioning food now, should I still do the increase to 2.25?
When Mae first started she was at 540, then 491, then 365 but those were all spot glucose checks at the vet. So she has improved over time but after all the reading I’ve done I’m sure it can get better with the food switch.
 
What percentage of calories is she currently getting from the DM vs. the FF pate? It'd be helpful to us if you could put a comment in the Remarks field for the day you start transitioning her to wet. Food changes can take a few days to see effect in the numbers.

She was at 60% dry 40% wet I would say and as of today I’d say it’s closer to opposite 60% wet and 40% dry. She eats the wet right away pretty much but there is some dry missing as well.
 
Her PMPS was 288 so I gave her 2.25. I’ll test her again in a few hours to see how that dose is. Today is the first day she’s been in 200 range all day so I’m taking that as a positive.
