My Vet refused now.. His BG is higher after Lantus

So we've been seen this vet for 3 months and he's very kind - he was willing to give us an advice without visiting him unless my cat was sick.

on Monday, we did usual BG curve. (Lantus, 3 unit, twice a day). I was really surprised that my cat's BG at 190 in the morning because he has been between 400-700 for 3 months (since the first time we found out he has diabetes). We thought maybe something was wrong, so we re-tested after an hour and his BG was 200. But I guess that made a sense since his number of urination kinda decreased this past 2 weeks (from 6 times to 4 times or 3 times even).

anyways, we fed him with wet foods and dry food (Glycobalance, only 20g) and started doing the curve. Then it went up to like 330... we checked every 2 hours. BG kept increasing and by 6pm, it was 670 and of course we didn't feed him between breakfast and dinner.

Any thoughts? Do you guys think my cat just stressed out by getting poked his ears??? (he is super sensitive, he actually threw up yesterday when we tried to check his BG again). Now, my vet just called me and told me to go see other specialist. Like internal medicine specialist because something might going on with my cat..

My cat seems normal, but we never know what's going on with his pancreas or kidney, that is true.

Any suggestion on this situation? I got Lantus bottle about 3-4 months ago , do you guys recommend me to buy a new bottle and check his BG? Or should I just go see a specialist? I dont even know how much ultrasound and all that's gonna cost... I even took this new job that I dont even like because I couldnt afford the hospital fee.

And about dry food, I tried to feed him only wet food and it didn't help either... And more importantly, he will throw up because he's so stressed out so I don't want him to stress out much. We already reduced amount of dry food so much, I do not want to take that joy from him. He's basically waiting 12 hours to have 20g of dry food that is even prescribed.

Have you guys seen case like this? BG goes up like crazy after insulin? Any advices?

Also, we quit Fancy Feast Pate because his BG got super high after we fed him FF pate. We switched to Glycobalance, then we switched to Reveral - and afterwards, his # of urination got decreased. He drank less than before too.
 
I don’t know much to help you other than my experience with Glycobalance DRY food which is about 35% carbs from the food chart I last seen.

My cat was on Glycobalance for months and I had no clue what it was doing to her. I started testing recently, stopped Glycobalance DRY and her numbers dropped significantly.

You can check my spreadsheet, just a small data point for your consideration. I don’t know why vets prescribe the dry food at all.

Note: the Glycobalance WET is about 14% carbs as well, so not that low in carbs either.

We suggest you test before every shot which you are not doing. Or are you but not recording results? If you can’t I strongly suggest a different insulin such as Prozinc. Lantus is dosed based upon how low it takes a cat and is a depot insulin and the depot build over time. The extremely high bg you have seen when you do a curve could be from the high carb food and/or your cat going lower than his body is used to and then bouncing high in response.
If his bg was high with fancy feast pates it’s because he was bouncing. I am not familiar with your current food.
