Maestro 10/1 update. He's now at 4U, help?

Good afternoon, my name is Carolyn D and my baby was diagnosed in April of this year. We went to three vets just to get him insulin prescribed. Once we found a vet who would, she admitted that she didn't know much about diabetes and that it seemed like I had all of the knowledge I needed and to contact her should issues arise. Four months into the treatment he was still having issues with a lack of interest in food, dehydration, ketones, and what the vet called indigestion. With added water to his food, Pepcid AC, and cetirizine he finally stabled out. I drug my feet in changing the dose because he was so unstable for so long. He had lost over ten pounds and was just fur and bones. He's gaining back some of the weight and his coat is looking better.

Now onto the chart. I did not start doing a curve until 8/28. I drug my feet, I wish I hadn't but all I can do now is move forward. I'm on the Facebook page and they recommended specific soft foods and some acceptable hard foods. I have three cats and all of them are grazers, so I leave three bowls out, Dr Elseys chicken, Dr Elseys Salmon, and Epigen 90. I tried young again but they wouldn't eat it. I give him the specific soft food three times a day, on the two days I work, he gets the soft food twice. He's eating more and definitely has more energy. He's playing again and even being social with us humans. I'm reading that 4 units is a large dose and I'm not really sure where to go from here.

I'm a full-time student, full-time caregiver, and a part-time nanny. I'm sorry for the issues with the chart. I really am trying to do better for him. I hope I did this post correctly.

Hi Carolyn,
The first thing I notice is you are only testing every so often. While you are testing is great, only testing every so often, and never at night, doesnt tell us what is happening all that time and a lot can happen.
It is possible Maestro is bouncing from dropping low, but we cant see it. It is also possible he does need more insulin, but, we need to see evidence of it. Do you think you could test before every dose and then test when you can in between the cycles?. Just testing once during a cycle is OK and then a before bed test.
If you could do that, we will learn a lot more abut what is going on.

Dosing for lantus is based on the nadir or lowest point, not the preshot BG.
Are you testing for ketones in the urine? With a history of ketones it is very important ketones are monitored. You can do this by buying a bottle of ketostix from Walmart or a pharmacy and testing a urine sample via the directions on the side of the bottle.
With a history of ketones it is also important Maestro eats plenty of food, drink well and gets his insulin. So finding out what is going on is doubly important with the history of ketones.

Thank you for responding! I am testing before each shot because I'm afraid of hypoglycemia and ketones. I have the ketostix but I also have the blood testing strips for it. I don't document the preshots but I do take them. I am starting to document everything down now. In the meantime do I keep him on the four units and just keep tracking his numbers, preshots morning and night?

Hi Carolyn, Sorry to take so long in replying....I have had trouble all day trying to get online.
Document every BG test you do. They are all important and are part of the bigger picture.
How often are you testing for ketones? Can you put the results of the ketones tests into the remarks columns of the Spreadsheet please so we can see at a glance?
Don't forget to put the dose given in the PMPS U column as well please. At the moment it looks as if you are only dosing once a day.
And also document those all important tests you get during the cycles into the spreadsheet as well please.
As you are following SLGS you will stay with the 4 unit dose for 7 days, unless Maestro drops under 90, in which case you would reduce the dose by 0.25 unit
