09/09 Fistuk AMPS 308, +3 347

I really hopeto see some positive shift soon. The constant stress is tough. Each time I feel there's a good trend a worrying shift follows.
It was a challenge to give SQ. He resists it again. I also hurt him, perhaps hit a capilary. It breaks my heart. He was screaming with the insulin shot as well. I'm sure the stress of it is not helpful.
Just needed to ket it out xx
And I'm feeling quite weary today....
Hang in there, Shelly! I know it's not easy at all - so stressful when they are unwell or you have to do something that's in their interest but it makes them unhappy. :bighug::bighug::bighug:

Let's see what he does today.

Thanks Bhooma. I appreciate the resonance and encouragement. I put the alarm to take a +11 but I couldn't get up.... everything you wrote is a yes :/
I'm taking comfort that he hasn't had any reds again and that there are no ketones. But each time I collect urine for a ketones test I'm anxious. I really pray for a shift. Thanks for being here for me :bighug::bighug::bighug:

I just saw his SS and that he's higher at +3. I think you can take him up 4.75u tonight. He's spending too much time over 300.
I know o keep asking the same question but I need to have the confidence and I don’t bc I don’t have experience …. It’s not too quick? It’s not going to lead to insulin resistance?
There’s no doubt that BG is not fun for Fistuk …

It’s not going to lead to insulin resistance?
What leads to glucose toxicity (a temporary form of insulin resistance) is him staying in high numbers and his body getting used to them and thinking it's the new normal.

With TR we increase by 0.25U every 6 cycles once a cat is no longer new to nadirs below 200. In fact, for cats with nadirs over 300, we increase by 0.5U. With Fistuk's history of DKA and tendency for ketones to develop, you want to fast track him to a good dose. We are only increasing by 0.25U every 4 cycles. And you are getting enough enough tests to see how low he is going on each dose, so it's not like you are bypassing a good dose.

Thanks, Bhooma. I appreciate your patience explaining this to me (over and over ;) )
He’s just owes in the flow. Which tells me he’s heading up to the red towers…
Il increase tonight.

So the SQ is a challenge again.
I was so happy when he got back from the hospital and it went so smoothly. But then each day the resistance grew and it's getting really hard again.

Today I tried to hold him in place with the towel and I managed to but it may have squeezed some of the fluids out. I'm not sure how much went in. I gave him 80. afterwards I noticed the towel was wet and it's hard to say how much went I'm. I tried to give him again some more but he really screamed. Both when the needle goes in and even more when the fluids start flow in in. I didn't manage to give more than 10ml.
I warm the fluids, I freeze the needle. I use 2'G perhaps I should try 21G but it'll take more time and it seems that the shorter the ordeal is for him the better. I don't know what else to do.

Let me tag @Bron and Sheba (GA) to ask about whether you need to give SQ every day when there are no ketones. Whether you can give it every other day or give less - like 50ml a day.

She will be asleep now, but will hopefully see the tag in the morning.
Perhaps every other day will be less of a harassment for him.
Yesterday was tough as well. I saw quite some blood when the needle was out. But I managed to get it in. I did it quite quickly and it was good. Seems like something's not working for him and I need to figure out what

Can you increase the fluids he takes orally? Add water to his food. Maybe even syringe water in his mouth if he will let you?

I'm so happy to hear this :cat:
He seems to be eating independently during the night but during the day it’s mainly with the syringe. I’m quite sure that it’s because he’s not used to eat so much and he was never a great eater. He has very good appetite and if he’s happy eating with a syringe at the moment I’m happy to keep it going. Just to make sure he gets enough food in

But is it the same? I used to do it and it wasn’t a problem for him at all. But the thing is that it goes through the digestion system so I’m not sure if it works the same. Also the composition of the SQ fluid are different than just tap water I guess
Yeah. For cats with advanced stages of CKD, oral hydration is not enough - that is why you give subq fluids. I don't know if it's the same with cats with a history of DKA/ketones. @Bron and Sheba (GA) will clarify.
