Do you have ondansetron a well as the cerenia?
How much food is he eating compared to what he would normally eat?
Have you managed to get a ketones urine test since day before yesterday?

no. I can check with vet if they have it

the most I managed to feed him was 280g which works at about 280cal.

Yes. Yesterday I think two tests )it's in the SS, started marking them with the BG levels) there are none! it's a good win. I hope we can managed to get his BG bit more stable
 
Try and get some food into him every hour. If he is clearing a bounce, you will see more downward momentum.
I gave him some at +1.5 then +3.25, after my call

At the moment the call with the vet feels like not what I expected. And honestly I don't have any expectations form doctors perhaps I should get the point that I should relinquish expectations from vets. But I'll wait for his plan as he said he's device one after he set with all the info. He knows nothing of TR. said in his "30 years for experience" he's had very little success with remission . Anyway...

I've had very poor experiences with vets both for diabetes and CKD. Fistuk is newly diagnosed and it takes a few months for some cats to be regulated. I'm sorry you are having such a bumpy ride on the ketones front, but hopefully now that you are able to give fluids at home, things might be more manageable on that front.

I'm wondering if you want to take him up to 4.5U tonight. He did see high blues for a couple of hours the other night but he's still spending too much time in higher numbers and we want to get him to a dose where he sees green quickly - especially with multiple episodes of DKA/high ketones in the past.

I was also thinking if we should raise the dose tonight, if you think it's not too fast.

He seems to be rather flat today - +6 391
 
I'll take a +10 today and see. yesterday he's gone to reds later in the cycle which I don't think it's good. But I really don't have experience here and I don't know what happens or could happen with increase earlier

With his history of ketones, you might be okay to increase tonight itself. He's not really "new to nadirs" under 200, so our aim is to get him to a dose where he sees nadirs below 100.

Increasing the dose:
  • Hold the dose for 3 - 5 days (6 - 10 consecutive cycles) if nadirs are less than 200 before increasing the dose by 0.25 unit.
    • if your cat is new to numbers under 200, it is recommended to hold the dose for at least 8-10 cycles before increasing.
    • when your cat starts to see nadirs under 100, hold the dose for at least 10 cycles before increasing.

Hi Shelly I don't think the vet knows what he's talking about.
It took me 2 years and 4 months to get Tyler into remission with the advice I took
from the experienced members here . He's been in remission since 1-24-21 knock in wood lol. So never give up hope :bighug::bighug::cat:

I don't give up because I believe deeply inside that it's possible. The call with the vet filled me with anger but it made it clear to me that I should save my money... I felt very supported and reassured right from the start by the community here and I just lean into it:)
