I know some people have had success with B12 but I think we're past that point? For the past few months, Boo has had some difficulty walking which got better, then worse, then better, etc. In the past few weeks it has continued to get worse, he's been missing the litter box, which we've cut down so he can easily step in it. Sat in random spots staring into space, used the couch less and less. As of yesterday evening, his back limbs stopped holding him up. By midnight, he couldn't stand at all. By morning, one of his front limbs wouldn't move. For the last 5 hours, he's been in the same stiff-legged type position and only occasionally lifts his head. I had to hold him up for him to potty overnight, and now he seems to have lost bladder control. His vision has been bad for years now, but since last night he doesn't open one of his eyes and really seems blind now.

I'm certain this is neurologic, whether from diabetes or hereditary or something else. He's 18 yo, bloodwork is good for an 18 yo (as far as I understand from some prior discussions here), diabetes is well controlled. He's had scans and has some arthritis in his back. He has a heart murmur which was scanned and both vets opined that is isn't causing him problems.

I guess I'm looking for support in making that most difficult decision. There doesn't seem to be much more that can be done to improve his condition.

I’m going to tag @Suzanne & Darcy here. Your spreadsheet in missing the top rows but I’m assuming if he’s on prozinc that you’re following SLGS. I see some progress there with the green and blues but I think your dosing is a bit all over the place. After he earned the reduction a few days ago, you reduced the dose to 1.5 which is perfect but then you reduced again at night to 1 and then back up the next day. I’d have liked to have seen you stick with the 1.5 dose. The methyl b12 only really starts to work once they’re regulated which I feel he’s close to now. Are you giving it to him daily?

Hi - thank you for your kind words. ❤️ The spreadsheet is from July 2020, after which we went to the standard shot protocol - same units 2x per day, with occasional curves to make sure he was regulated overall. It's worked very well, so the bg's aren't a concern. ( I should probably remove the link to the spreadsheet).
 
There's a chance some one may tell you Boo's life is precious and is worth saving. Maybe it's just a deficiency or a dose adjustment.
Our story was Jasper, an old and proud cat now confined to the bedroom who couldn't even crawl to the litter. After an MRI and a spinal tap he just laid there, "why don't you just let me go". It's not about diapers and one more year of a miserable life, it's Boo's dignity. At 18 he's had a good life and now it's up to you. Have a long talk with Boo and see what he thinks, you'd be surprised what a cat can tell you. Just my opinion, I'm not there.

Just wanted you to be realistic. I know what's in my living will, I want my wife to get on with her life and not spoon feed me tapioca pudding and wipe my behind for the next 20 years. Would Boo want that just because it was possible? Maybe it is something simple but if it's not put yourself in his place.
 
Update: Thank you all for posting. We met with the Er doctor to discuss quality of life and she agreed that there was very little chance he would ever walk on his own again. So he has crossed over the bridge. This bestest boy has been my 20yo son's buddy since son was 2. It was a very, very difficult day.
 
Crap! Just found out our oldest cat may have cancer and now this.
Please take care of yourself and know that Boo knew you loved him.
 
Sooner or later too many people start to wonder if only they had done this, fed him that, I should have known earlier. Please don't let guilt creep into your life, you gave Boo a wonderful life and you both deserved better.
One day another tiny paw might reach out to you but there will always have been only one Boo. Turn your phone off, cry, eat something bad for you, get some sleep and more than anything else be kind to yourself. That's what Boo would have told you.

I'm so sorry for your loss. As hard as making that decision is, you did what your heart and caring for Boo told you to do. It was a final act of kindness.
