6/17 Bella AMPS/23.5, +2/21.2, +4/16.3, +6/16.7, +8/16.4, +10/16.3, PMPS/16.1 Curve Check

Our second curve! 1.5U dose this morning?

AMPS/23.5, +2/21.2, +4/16.3, +6/16.7, +8/16.4, +10/16.3, PMPS/16.1

Are these strange numbers?

I’m going up to 1.75U or stay at 1.5U???

Sorry no one responded earlier. If possible, you want to get some "spot-checks" on non-curve days. A test before bed at night or the stray test if you are home etc. If she is bouncing on the day you decide to do a curve (from lower numbers in the last couple of days), you would get flat and high numbers. So too much insulin could look like not enough insulin.

Good luck with the increase! :-)

This is hard. My vet thinks I’m over testing doing the spot tests. I was doing the +4 at night but her numbers were all over the place and I wasn’t sure what benefit I was getting if the numbers just seem so all over the place.

+4/6.6
How will I know if too much insulin could look like not enough insulin?

Why does your vet think so? Does s/he think you are hurting Bella by testing?

Preshots often do not reflect how low a dose is taking a cat. Newly diagnosed, unregulated cats can start a cycle in the high teens or 20s and drop to single digits at some point and then bounce back to double digits by the end of the cycle.

Take Bella's example - if you hadn't gotten those tests on 28/5 where she dipped to the 2s and only run a curve on 29/5, you would have thought that she isn't getting enough insulin and she needs a higher dose. Whereas, actually, she was bouncing on 29/5 from dipping very low on 28/5 and was actually getting too much insulin.

I don’t think the vet was saying I was hurting the cat, I’m not sure why but he did say that we assume the curve should be mirrored AM and PM but I know that’s not happening it’s all over the place hence bouncing. So I think the vet is suggesting I change to canninsulin because he isn’t sure about glargine, how the slow release effects the curve/ If the AM dose is out of her system by her PM dose. I have a grazing cat that does not eat on demand so as I’ve research more, I won’t be swapping insulin at all.

I’m was finding the numbers confusing and when the vet said I’m over testing I stoped maybe because I needed a break. However, that example does really help and make sense. (Also that example would have been residual effects from when my old vet had her on 4U as well). I’m learning my new vet doesn’t know much either! Above all I want her to be regulated and to be ok.
I’ll work on trying to do the +4 PM and the random +4-8 AM when ever I can. Would it be annoying if I ask for help interpreting the data?

You don't have to stay up or get up at +4 at night to test every night. Not all cats nadir at +4. Also, nadirs can and do change from cycle to cycle. So any tests you get is good info.

A test at +2 can act like a crystal ball of what the rest of the cycle is going to look like.
  • If the +2 is about the same as the PS, it's usually going to be a pretty normal cycle - gradually down to nadir and then gradually back up to the next PS.
  • If the +2 is a lot higher than the PS, that usually means there's a bounce starting and it's usually OK to take a little bit of a break from testing.
  • If the +2 is (a lot) lower than the PS, that's your early warning that it could be an "active" cycle and numbers might go a lot lower later in the cycle and it's important to try to get more tests.
Of course, some cats have an onset later in the cycle like +2.5 or +3. In that case, a test at +2.5 or +3 would be a better indicator than a +2.

We are here to help and answer any questions you have - so that's not annoying at all. FD can be very overwhelming initially and quite baffling. We have all been where you are. With proper dosing, she will settle down. Some cats can take longer to get regulated. I would not switch from Glargine to Caninsulin.

This helps sooooooooo much!!! Bella’s nadir seems to be +4 but it also moves, so no idea at this stage and we are in the early days.

so doing a +2 at the PM would be better and I can do AM Wednesday., Saturday and Sunday when free as well as my curve on the weekend.

you have been so helpful - this has given me a better understanding of how to interpret the bounce and what my response should be!

@Bandit's Mom
Thank you so much! This is just so helpful! PMPS/17.7 and +2/17.2 so good just getting a better understanding! My little girl is laying on me purring, content and I’m feeling much better with this information.

Patterns can be hard to spot in unregulated cats. Just collect as much info as you can. For now you just want to get an idea of how low a dose is taking her, so you can adjust the dose accordingly. In cycles like tonight where she is high and flattish, you can just call it a night.
