5/29 Remy AMPS 144 (looking for general dosing advice)

I haven't posted daily in the Lantus group but wanted to get some eyes on Remy's progress so far. He is currently on 0.25f and has been very stable. We are using AT2.

Any recommendations in terms of dosing? Do I just keep him at this dose? I've been giving him a shot as long as his PS BG is above 100.

Thanks!

Remy is looking good. I'd just like to see more green on his spreadsheet. I'm tempted to suggest increasing the dose to 0.75u.

I also agree with Elise. It would be helpful if you can get at least 4 tests per day -- your 2 pre-shot tests and one additional test each cycle. I'd also encourage you to get at least one test during the PM cycle every day.

You are seeing nice flat cycles. However, I think he’s a good candidate to go otj. Would you be able to test two times every cycle? If still feeding all wet food you could switch to TR. If not how how could you test? To go otj he needs to be mostly 68-100 with the AT. @Wendy&Neko @Sienne and Gabby (GA) @Bron and Sheba (GA) @Bandit's Mom Thoughts?
I am in the office most days of the week and am gone from +1.5 and come back almost at PMPS shot time so during the day it would be quite hard.

Question for the numbers you referenced (68-100)... I thought 80 was the take action number on a pet meter and 68 would be hypo? did I get those numbers wrong?

I would love for him to go OTJ but wouldn't want to put him at risk. Is there a simple explanation as to why bringing his numbers so low would help with remission?

Thanks.

When he was at 0.75u he was dropping over 200 points in a cycle which I thought to have read somewhere that this meant the dose was too high? Could I move him up to something between 0.25f and 0.5 - say 0.25 medium and see how it goes?

Pet meters read higher which is why I quoted slightly higher bg. You would want the bg mostly under 100. The dose is not too high if the bg drops over 200 points v dosing is based upon how low it takes your cat. With TR reductions are earned if under 68 on the AT. With SLGS reductions earned if under 90.

You are seeing nice flat lantus cycles. Any chance you could switch the insulin time 30 minutes earlier so you could get a +2? That’s when Lantus typically kicks in. If lower than the preshot you would feed slightly higher carbs. If you can use an auto feeder you could set it to go off at intervals while you are at work.

Thank you.

He does have an automatic pet feeder but not sure how much he or his sister uses it, but I do leave food for them when I'm gone for work and have it move to an empty position at +10.

I can do his insulin at 5:30 am and get a +2 at 7:30am but that would leave me very tight to get home and give insulin at 5:30pm as I tend to get home 5:45 or 5:50. Would it be ok to always be late on the PM dose by 15-20 minutes?

What dose do you recommend I try? Would I be able to slowly increase to see how he reacts? When he was at 0.5 I had to reduce as he dropped under 90 and had to feed him to keep him from going lower. Would a 0.25 medium be a good next trial?
