It's good you switched to TR. You can get him to a good dose sooner. For newly diagnosed cats, proper treatment can help getting them regulated faster and even increases their chance of remission. With TR, you would increase the dose every 3 days till you see nadirs under 200.

Hi! The antibiotic he's on is Unasyn, he will be on it for 7 days. I haven't heard of S. Boulardii here in Venezuela, but I could try looking for it
 
When should I test again? I'm super sleepy but I can set up an alarm. I could aim for a +6 or +7.

With Lantus (Glargine) a +2 is usually a good indicator of how the cycle is going to pan out:
  • If the +2 is around the same as the PS, it's going to be a pretty normal cycle....gradually down to nadir and then gradually back up to the next PS.
  • If the +2 is a lot higher than the PS, that means there's a bounce starting and it's OK to take a break from tests.
  • If the +2 is a lot lower than the PS, that's an early indicator that numbers might head a lot lower and you need to get some more tests later in the cycle.
He's not much lower at +3,s o I would just call it a night.
