I’ve gotten to the point where I can test Pumpkin fairly reliably. I tried a curve on Friday and got all the measurements I was aiming for except one. So far, her blood glucose is pretty stable near 300. She’s continuing to drink a lot (16 oz per day) but it’s maybe starting to trend down. She’s at 1.5u BID glargine

Pumpkin’s currently eating a 40% carbs hydrolyzed protein diet due to a food allergy (we haven’t narrowed down the exact allergen). Our first attempt at a diet change failed with the return of vomiting.

At her last vet visit, some dental issues were noted, but she was going to need sedation for a full exam and either cleaning or tooth extraction. We paused on that to take on the diabetes.

So that leaves me three avenues to try:
- Dental work
- Dose increase
- Diet change

I now have some Tiki Cats Luau Chicken, which is basically just chicken and vitamins. She tried a few samples last night and this morning and loves it. I’ll keep going with samples before trying full meals. But, I’m ready to try that next in a couple days if it makes sense.

Or we could try to get her dental issues fixed so that that isn’t raising her BG. It’s currently not affecting her appetite at all for what it’s worth.

Or we could some more dose increases until he start seeing her BG come down during the day.

What should I be thinking about as I weigh the different options?

Well done getting more BG tests.
Try and get all the pre shots if you can and at least one test in during all the cycles. The pm cycle is just as important as the am cycle and cats often drop lower at night. A before bed test is a good habit to get into, and if that BG is lower than the pre shot, that can mean an active cycle and the Bgs could drop further, so I would get up and test again to see Pumpkin is in safe numbers.

I think you could safely increase the dose to 1.75 units.
Holding the dose too long when it needs increasing, will only encourage glucose toxicity.
You suggest options. Whatever you do, only do one of those options at once.
If the teeth need work doing to them, I would not leave it too long as that could be contributing to the higher BG numbers.

Thanks for taking a look! I know there are a lot of threads to keep up with.

We had another mucus vomiting incident this morning. There was also one four days after I ended the last food trial which seems just on the edge on when it could still be inflammation from allergies. So I’m not sure what’s going on but I’m going to keep easing slowly on the diet.

I’m talking to the vet again at the end of the week so we’ll start looking into dental then.

Meanwhile I’ll bump her up to 1.75u.

I’m starting to do more evening tests now. She’s spicier in the evening, so I’ve been saving those for last. It feels like we’re finally starting to get a system down enough that we can make it work in the evening too.
 
She now has a 3:30 am meal since she’s up anyway and asking for it (this is new since the diabetes) so there’s isn’t really such a thing as before food anymore. 11:30 pm -> 3:30 am and 3:30 am ->7:30 am are her longest gaps though.
That’s good. I thought she might have been vomiting because she had an empty tummy.
