Jekyll pt. 2 bun/crea 44 kidney issues?

We are at the vet now. I relayed all my info and wrote everything down I can think of and put it with his care bag. I requested cbc, electrolytes, snap fpl, and possibly a nose culture. Let's see how much of this I can actually afford lol!

I'm terrified that they won't push insulin right at 10am in 49 minutes so I'm staying in my car and going in at 10 to make sure. This is a drop off appointment :(
 
I always used to feel so extra for doing stuff like that, but then when I didn't they would ALWAYS mess something up or forget something (sometimes even having a list...), so now I don't feel bad anymore.
They can roll their eyes at me all they want, I'm paying them and have the right to make them deliver the services they say they'll perform lol

I'm usually very much uncaring about how i am, but this specific vet will give me discounts when I'm registered with the department of state! so I need them to kinddaaa like me. With other vets I don't care at all!
 
Well I spoke to the vet on the phone and all of jekyll's testing which included a CBC electrolyte panel and chem 17 came back good except his lipase is still elevated and of course his glucose. He wants to try azthromycin for the uri symptoms.

I let him talk me into a urinalysis even though I'm pretty sure it'll be fine. Over $400 so far

Not too bad of a bill, my Vet would have cost more! If his chem was good that rules out alot of other things, it could just be a really bad URI.

Good luck with the new abx, if the previous owner didn't administer insulin, I'm sure they weren't diligently giving other meds.

Jekyll is in good hands with you, you can help him heal. And from experience with my last diabetic kitty, any infection can cause high glucose, so just be calm and let the magic happen.

Do you think infection can hide in a full bloodwork? They can't see anything

Don't forget that you were giving him antibiotics for 2 weeks... So that's going to affect the LABS.

Also... This could be viral, so there may not be an active bacterial infection, it could be a viral infection. Were his Eosinophils elevated at all?

I'm not a Vet, but that's high end normal, and in line with inflammation. Its probably whatever sinus thing he has going on.

I would do the antibiotics and like another member suggested, take him in the bathroom with you during a hot shower, or just run the shower and get it steamy for him in there. It definitely will help him if he's stuffed up. And the B12 injections will help. You'll need the correct syringes for that, too! It's Subq, just like insulin and fluids,

Oh yes he gets the bathroom treatment when I take my daily stress bath lol! I also have a cool mist humidifier in his room :) I just have to confirm my vets b12 is methyl. Anything I should keep an eye out for with the azithromycin?

I've only used it a few times. Google package insert and you'll find the pharmaceutical information about side effects, etc.

Ah OK, I thought for some reason you had first hand experience. I'm just sleep deprived again lol

Do you think some cats just do better on different insulin? My vet mentioned igf-1 testing already and that's got me a little freaked

It's been awhile since I've used azithromycin, I have more experience with Clavamox, Clindamycin, Zenequin and Doxycycline!

I can talk about things I never wanted to know about regarding cat health!:eek:
 
Maybe the elevated bun is from the high protein dr elseys? And maybe it was elevated in November from vomiting/dehydration? That's why he was taken in back then

No. We don't do IGF -1 testing until the cat is at a much higher dose than Jekyll is. Don't waste your money at this point (and this is speaking as an Acromegaly cat mom here!)
 
Well I'm reading his bloodwork and his bun is very elevated. his bun/crea is 44. Not happy
Creatinine is a better indicator than BUN. Jekyll's CREA is within range. Yes, high BUN is not good and we all know that, but there are many factors that do influence BUN. Did they do an SDMA? Unless he's lost a lot of weight recently? That can cause reductions in CREA.

Nope I have all my results. I posted them above :)

Definitely vomiting/dehydration will elevate BUN. It actually elevates a lot of numbers (HCT, proteins, increases USG, etc.)
His current USG is normal low. His pee is almost clear too, this man likes to drink

So I didn't do the nose culture. Vet says of this azithromycin for 5 days doesn't clear whatevers left that we should try myco/herpes/calici panels.
I like this vet being more proactive about trying to find out the cause! I’m sorry about the expense though.

Usually we like to give ProZinc a few months first before recommending switching. After that, I would try Lantus if you aren’t seeing better BG control.
 
He said he wasn't too worried about the lipase because he's eating and drinking fine now. They also didn't have snap fpl there so it'd have to be sent off :/

Does anyone know about advita probiotics?
That sounds reasonable. I know the spec fPL has to be sent out. Glad he’s eating and drinking.

Ok I'll hold out! My other cat went otj on prozinc and I had a bunch of u40s left so I thought I could start prozinc cheaper, boy was I wrong. My first vet refused to give us the insulin while he was DKA and I spent 500 just to finally get insulin.

I syringed him for 2 weeks to get him out of dka, now that he's better he loves food again! I think advita is just cheaper forti flora, I'm trying to get him a short supply of proviable since mine just expired and is clumpy. Idk if it'll still work expired but my vet doesn't have probiable apparently

I think this vet was surprised by my knowledge in general. When I met him he told me to update him and I gave him commentary access to my SS. I told him I'm a little crazy on BG checks so he can check it anytime and have a current update. I am not chill about sick cats lmao I ask a million questions, I said after this round no more empirical treatments, and he was very cool with that. Helps that he prefers glargine insulin so I respect him more than the vetsulin and nph cronies.
 
If Advita is as attractive to cats as Forti flora and it’s cheaper, I should look it up. I mean, Forti Flora usually gets my cats eating - not a great probiotic but they love the taste.

Glargine is good insulin. I do think that’s good that he seems a little more up to date on stuff.

He looks about my age or younger so a recent graduate I'd say. I'm excited to show him this group! He said he's only ever seen one cat go OTJ, maybe this group can help him and his patients :)
