Can anyone pls explain why BG values go up after an insulin injection? It’s been a few days now where Tigera’s Bg increases after her insulin ☹️I cannot get her regulated and I am so upset. The only different thing that’s been introduced is semintra. But her vet actually said that the semintra will help lower her bg to the point I should watch for hypo. Yet her bg numbers keep going up. I haven’t missed any shot - there hasn’t been any fur shot. She is on 3.75U of lantus pls see her chart
 
Hi Antonella, you are holding doses too long. I'm not sure which of our two dosing protocols you are following, but even with SLGS, you do not hold doses longer than a week unless it is a good dose. The rationale for not hanging on to a dose if numbers aren't where you want them, is that there's a tendency for glucose toxicity to develop. The term sounds a lot worse than it is. What this means is that your cat's body gets used to being in higher numbers and treats it as the "new normal." This makes it harder to get the numbers back to the real normal and may contribute to bouncing.

It is good you increased to 3.75U. If you can get at least 4 tests per day - 2 preshots and 2 mid-cycle - you might want to think about following TR - at least till you get to a good dose. Once Togra starts seeing better numbers, you can switch to SLGS if you aren't comfortable with TR.

That’s terrible of me. I don’t know what I am doing ok I should change the dose after 6 cycles right ? If there is no improvement? But how high do I go !? She keeps needing increases. It’s so scary. It’s like nothing is working. I don’t understand how to navigate the SLGS & TR . I don’t know how each of those work? It’s not always possible to do the mid days checks as I work and I’m not home. Im also a single parent so no one is around to test her other than me. Thank you for responding now that I messed up increasing her dose , is this going to be very hard for her to get better numbers ?

Not necessarily. On SLGS, you hold the dose for 7 days and then do a curve (test every 2 hours for 12 hours or every 3 hours for 18 hours). If you can't do it at exactly 7 days due to your work schedule, maybe do it on day 5 or 6.

Every cat needs however much they need so please stop worrying about her dose. If you follow the protocol, at some point she may break through that glucose toxicity and start earning reductions.

She has a lot of health issues besides diabetes which makes it that much harder to get her controlled so please know that you're doing the best you can and we understand that.

Try to post here daily (or at least every other day) and get some more experienced eyes to help you.

If she does go over 6 units twice a day, we will probably suggest you get her tested for acromegaly and IAA but she has a way to go before we get that high.

Hopefully the IM vet you see tomorrow may have something helpful to offer.
 
Thank you Chris… somewhere along the way I forgot how to follow the SLGS AND TR and haven’t done any curves as her numbers were always high. Her appt with the specialist is not tomorrow it’s on Jan 6 and I will keep you posted . In the meantime - how can I fast her for 14 hours ? Isn’t this inhumane ? She is diabetic and wants food. I don’t know how I can do this . Also , if I do fast her , what about her insulin injection in the morning ? How much do I give her since she will be starved and no food ?

If you are talking about doing a curve you don't fast them when you do a curve
You only withhold feeding 2 hours before the AMPS test as usual
You go ahead and feed her during the curve as you should

I was talking about her specialty vet appt scheduled today… the instructions were to fast her 14 hours and it’s been so sad as she is crying for food and stressed
