Pringles change from prozinc to Lantus. What dose to start?

Hello,

Pringles rescue wants me to change from prozinc to Lantus.
My question is to what dose to start on.
He is on 3.5units prozinc, but was thinking to start at 1.5 or 2 units Lantus.
Any suggestions?
His spreadsheet is in my signature.

When people switch to one of the depot style insulins (Lantus or Levemir), we take into consideration the existing dose and how they are doing on it. There's not a lot of mid cycle data with Pringles on 3.5 units, so it's a bit tricky. Our dosing methods suggest doing a curve before deciding to increase. Could you get a curve before switching? If the lowest you are seeing is the 200's, you'd be fine going to 3.5 units.

Do you have the U-100 syringes for Lantus?
 
I agree with Wendy....can you do a curve tomorrow? Test every 2 hours for 12 or every 3 hours for 18?

Don't forget we have clock's falling back overnight tonight too.

Hi,

I was planning on doing a curve for all 3 today, so I can definitely do a curve before switching.
Yes, I worked thr whole week towards changing their feeding and insulin time.

And I have the new U100 syringes.

Do I report back here on this threat or do I start a new one?

I would save the new thread for the first day you start Lantus. Good luck with the curves.

I did Pringles' curve today.
I updated his spreadsheet, but for convenience:
AMPS: 367 - 3.5units ProZinc
+2 414
+4 398
+6 389
+9 332
+10 425
PMPS 341 - 3.5units ProZinc

I assume i have to start on 3.5units of lantus as well?

I tried opening the spreadsheet, but it looks like the permissions have been modified, so I can no longer see it.

Can you monitor Pringles closely the first couple of cycles on Lantus? If so, then I think you'd be OK with 3.5 units.

Yes, I can check him closely. I will test, +5, +6, +7 and a +4 at night
 
When you start the Lantus, you should put a blank line on the spreadsheet before the day you start, and label it "start Lantus" or something like that. It makes it easier for us to start the change. Good luck with the change!
