10/21 Pippin AMPS 281, PMPS 162 Skip? Adjust?

So Pippin ended up getting down to 162 PMPS. Should I feed and test again in a bit?

Edit: I got busy with work and couldn’t test in the afternoon. Should his dose get adjusted down again, or do I need to wait longer and see?
 
Could you fill in the dose for last night on the spreadsheet. If you delay, do not feed, wait 15-20 minutes, then test again to see if he is going up. If you feed, that will bump him up and you don’t know if he was going up on his own.

Oops, sorry, thought I did fill in the dose, just did it now.

I did end up feeding Pippin. Should I not shoot, then?
 
Bumping this, just wondering if I should skip the dose altogether by this point or give him his dose? I haven’t tested Pippin because I did feed him earlier.
 
How long is is since you fed? And how long since the shot was due? Can you get another test now?
He got fed about 1.5 hours ago, usually he gets his shot at the same time. I can try to get another test in, but he was mid-100s earlier, and the food should have boosted him a bit.
 
I saw a reference to a food change in a previous condo but can't find more details. Maybe it's a few condos back. Was he recently changed from a high carb diet to a low carb one? If yes, when and what? As Wendy pointed out, that can make a big change to a kitty's insulin requirements.

He was on dry diabetic Purina DM, but he swapped to only wet Purina DM about a month ago. He used to be on 5u, we’ve been trying 3u but it seems like his BG never raises again, just keeps dropping between doses.

If he’s above 200, should I shoot? Additionally, would I need to feed him again, or is the food he ate before good enough (I don’t want to give him insulin poisoning by not having enough in his tummy).

I think it's almost 2 hours now since his shot was due? That would mean your future shots will get delayed. Is that something you can accommodate? Let's see where he is?

A small housekeeping request - from your SS, one can't tell if you started Lantus on 10/17 or started hometesting on 10/17. I read back and figured it was the latter. if you could add a row before the one for 10/17 and say something like "Started Lantus on MM/DD but started hometesting on 10/17/2022)

Yes, I can do that, also I can start slowly scaling him back to his normal time.

usually he gets fed right before each shot, so twice a day.
also, yeah, I can try and update the spreadsheet for more info. I just started home testing a little bit ago, that’s where it starts :)

You want to feed a couple of small meals during the cycle so that he doesn't dive with insulin on board. You could start by breaking up the meal he gets with the shot into 3 and feed 1 part with the shot, second at +2 and the third at +4. Many folks here use a timed autofeeder that will make food available to the kitty when they are not at home or asleep.

I was still trying to decide, but I think I will. I really don’t like how many doses Pippin has skipped this week, but it’s better to be high than low.

I’ve looked into auto feeders, but it gets tricky because we do have a second cat. Usually we have to separate them to eat because she’s not diabetic and still is on dry food. I will see if I can split at least the meals into a few portions from now on.

Btw, if you can get some mid cycle tests, you can get an idea of how low he is going on a dose. Say a test before bed or a test before leaving for work (at least 1.5 to 2 hours after the shot). Some tests over the weekend etc. Cats often go lower at night and bounce in the day.

There is no harm if your non-diabetic cat eats low cab wet food. In fact, it might be good to transition her as well so that you needn't worry about Pippin getting into her food. And dry carby kibble isn't good for cats anyway. If Pippin is microchipped, the feeder will only open for him.

ETA: A skipped shot provides you with the opportunity to change your shot time going forward. A skip tonight means you can shoot at any time tomorrow morning. You could choose a shot time that will allow you to get a test before leaving for work and before going to bed.

Okay, thanks, I’ll look into auto feeders again. I’ll try to get more testing done for the middle of the cycle as well.

And yes, dosing advice would be appreciated. I’m wondering if I should drop down to 2.5? Or even lower than that? But I don’t know if that’s too much of a change too fast or not.
