**UPDATE: Wendy has been on 4 units for 12 cycles and as of 10/02/2022, we haven’t been in the red. I haven’t posted in a few days due to attending to a family emergency, but I finally was able to update my spreadsheet so I can get some advice from the experts.**


On 10/04/2022, she got “fur shots” on both cycles. The PMPS was administered and shortly after the insulin leaked out! I advised the vet and he said it’s nothing to worry about. What could of happened is that I didn’t inject the syringe needle all the way and insulin leaked. Could that be possible? As far as I can remember, I did inject the needle all the way in and I’m a bit perplexed. I tested for ketones and there was a small trace which was addressed with the vet. He wasn’t too worried about it and recommended to continue administering the sub-q fluids.


10/06/2022, I applied the Mirtazapine ointment on the inner ear. About an hour later, Wendy threw up twice. The first time it looked like her wet food and the second time it was clear. She threw up back to back and I was very concerned because it was a good amount of fluid. I then called the vet and he suggested we give her Cerenia to help with the nausea. He thinks Wendy probably cleaned her ear and ingested the ointment. I administered the injection at 330pm along with sub-q fluids and fed her with a syringe. Wendy was taking small bites of her wet food throughout the day, but not enough. She was alert and walking around without any issues. However, she did look a bit weak and not acting like herself. I tested for ketones and there was a trace.


10/07/2022, I applied Mirtazapine ointment on the inner ear after AMPS. Wendy ate a full can of Fancy Feast Savory Centers wet food 2.64oz. and has been eating small meals throughout the day. She’s been alert and moving around. She also looks more energetic and has not been sitting next to water bowl. I also administered 50ML of sub-q fluids. I tested for ketones and there was no trace.



Experts:

Do you recommend increasing the dose? The vet recommended leaving her on the same dose, but he also said if I want to increase, to do so. Do I continue to apply Mirtazapine ointment for the full 14 days? Is it normal to see small trace of ketones and then no trace? Do you recommend I do further testing? If so, what test and/or test’s do you recommend I ask the vet to perform? I currently changed her food to Fancy Feast Savory Centers Wet food since that’s all she will eat. I can’t tell if it’s a high carb food and want to make sure I’m feeding her the right food. Would you be able to tell me if this food is okay for me to feed her?

I'd take her up to 4.5 units tomorrow. Normally we'd say increases by 0.25 units with yellow nadirs, but I'm seeing that she got some ketones above trace at this dose. Let's see if we can get some blues in the picture, and put ketones in the rear view mirror! Ketones can vary day to day, which is why we suggest daily testing when you've seen them recently. They can also increase very quickly.

Sitting by the water bowl a lot and not eating as much as she should are signs that she might be nauseous. A better drug for nausea is ondansetron. It's a human anti nausea med that you get at regular pharmacies, but your vet has to write a prescription for it. Cerenia definitely is good for vomiting, so maybe do both for a bit and see if that helps? Make sure you give the anti nausea meds first, before the Miritaz. You don't want to give an appetite stimulant to a nauseous cat. That can cause food aversions to develop. I would give all meds for a few days to make sure she's eating fine, then remove one at a time, maybe starting with the Mirataz.
