AMPS 246 /Kaz - increased insulin 3.25

Just letting everyone know that I increased Kaz's insulin to 3.25 this morning it seemed necessary.
I saw the vet on Friday for both cats. Kaz has slight anemia but they said that overall he's doing well and that I don't need to do anything for the anemia.

Other cat has kidney issues which is my next stop to do some research on this.

I have requested the blood work from the vet but have not received it yet.

Really not much else to report except the vet told me Kaz is way over weight and that this would be hurting him going into remission so now I am trying to figure out how to reduce his food as he's supposed to get the three small meals in between the the two normal meals. I'm wondering if I can go to two small meals versus three as this would cut down quite a bit.
Don't remember why it was you had put me on the three small meals so if anyone can remind me. I will also go back through the old messages to try and figure it out myself.

Thanks that is what I started doing today.

On a side note the cats BG dropped to 43 and I only just increased the dose from 3.0 to 3.25 this morning. What does this mean??

Do I now reduce it again to 3.0 as the number is below 50. Bit puzzled with this one.

Sorry follow up to the feeding question. For his two mail meals he gets a full can can I reduce this to 3/4 can or is this not a good idea?
 
Are you feeding a can for breakfast? How much at the mini means? How much does he get each day? That sounds like a lot of food. You want gradual weight loss so you could give 3/4 a can at amps. How much at the next two feedings? Do you have a digital baby scale to weigh Kaz? If not I would get one.

Did you recheck that bg? It doesn’t seem like it’s right. You would reduce if it’s correct.
Just rechecked now this is the time I normally do blood test prior to next shot. The number has come up but its still only 84.
So I am guessing I go back to 3 units but shoot the full 3 as the number is above 50.

Yes he gets a full can at breakfast and dinner. Then he would get 1/2 can (lg spoon full) for each meal). He's basically eating 5 cans a day (small cans). But way too much as hes 2 1/2 pounds over weight.

Now I am trying to take a small can and split it into three meals which means I would go down to 4 cans a day. Guess I need to do this first then recheck weight in couple weeks and then maybe reduce the two main meals to 3/4 can each.

If I don't hear back within next 15 minutes I will reduce to 3 units again. Not sure it would be safe to give him 3.25 at his current level. 3 units makes me uneasy but I told everyone I would shoot the full dose if 50 or above.

Means I will have to stay up with him all night or at least till I know he's safe.

Yes I would reduce the dose to 3 units after the 43
And seeing the PMPS was 83 I would get a +1 and a +2 to see he is not dropping again. You may need to test further after that.

I did reduce and I rechecked at 2 hours hated moving him as he was so comfy but his numbers have increase to 119. I feed him again in about hour so I'll check again now. He's not happy right now as he associates ear pricks with food so he does not understand why I'm not feeding him.

Can’t you give him a small treat each time you test him? You need to keep the faith!

He gets a treat before every prick or insulin shot and as I did not feed him I gave him a treat after but he still looking for food. He’s been sitting at the kitchen door since the earprick actually lying there now.
 
Hi Claire sorry about your babies diagnosis, I know that is even more stress added. I agree with Bron keep the faith. Kaz will get used to it, it will just take some time. I give Panzer & Spook a boiled chicken treat everytime I test & it keeps them going. I test alot as you can see on my spreadsheet. Just to give you an idea I feed 4 1/2 cans between the 2 of them total in a 24hr period. That's a 3oz can of FF every 6hrs. They get 1.5 oz between the 2 of them @+2 ,2.5,3 just depending on the day. I was feeding them every 4hrs 6 cans between the 2 of them. Since changing there schedule & making them more hungry. Panzer has gotten 2 reductions & they are both off of the Mirataz the ear appetite stimulant & they are down to 1 ondansetron each a day from 2 a day. I never knew feeding them less could help them become well, but it has. I boil a chicken breast let it cool & split it up into Tupperware bowls & use as needed. Everytime I test they get 2 or 3 little bites of chicken so that supplements there loss of food in the evening & when I get home from work. They are both playing, running, & feeling better. I know alot of people can't test as much as I do(I'm an overtester) Lol, so you could just do lesser amounts of food throughout the day & night. Then kaz can drop some weight & possibly get some reductions. If I had known sooner that feeding less works for my boys & I get more sleep now too, I would have started this months ago.

I know I need to reduce the weight the 8 meals a day is the issue but right now that's what I guess I need to do but I just need to cut the amount down slowly. Kaz has actually started sleeping on a high leveled bed which he has not been near since he was diagnosed with diabetes so he must be feeling better. I also found him at the open window two days ago another first since the diagnosis. He obviously hasn't felt like he can jump up here safely until this last week so I am hoping that this is a change in the right direction.
 
Sorry I didn’t get back to you. I was waiting for a call from my vet and then got a workout in. That low bg must have been accurate. Congrats on the reduction.
 
Also don't think Kaz would tolerate that many ear pricks but your sheet is pretty impressive. Lots of greens that's what we are shooting for.
Panzer is not always cooperative (he sometimes scratches me up)but he's used to it & loves his chicken treats & of course he gets his cuddles & good boy when I take him down out of his test box. He gets up in his box on his own, I just say test time & he runs into the dining room & gets in his box & waits for me to test. I move the chairs out because brother Spook thinks he has to be
up there when Panzer tests :smuggrin: he wants to play with Panzer's tail & Panzer doesn't like him up there Lol. He looks forward to the chicky treats, they both still head towards the feeding room sometimes but there adjusting pretty well.

@Claire and Kaz this will only help if Kaz likes soupy snacks, but you could try adding warm water to his canned food meals. It tends to make them feel fuller, and tricks them into thinking they got a bigger meal.
That’s what I do with Chewie’s “anti-hangry” last snack at +9 every day. Put maybe a tablespoon of food, then two tablespoons of water from the kettle. Helps make food more appealing by making it smell stronger, and keeps her from howling until PMPS.

I do add water to both cats food but not as much as I used to as now he tends to drink more although his water consumption has dropped again which I am assuming is the insulin kicking in. Less drinking I associate with good things now for Kaz.

You might have been able to prevent that drop if you had an earlier test and if dropping fed higher carbs. Some cats respond in a major way to the first or second day with an increase. You want to feed the curve to prevent a drop then. I suspect t you will be right that he will need the increase but let’s see.

I like to do increases on Thursday evenings as I am usually around to test more on Fridays and the weekend.
I know I am supposed to hold a few cycles at the 3.0 but if his numbers now go back to where they have been would it be ok to adjust again on Thursday dose 2? I do have an engagement on Saturday but should only be out for about 4 hours so I can keep a closer eye on him.
 
Also betting he was in lime green earlier in the day. Not really sure what is suddenly changing here bit strange but good news.
Only difference in last 48 hours is he had a rabies shot and that wouldn't do anything.

Something odd with his numbers right now and I can't fathom it out. I was expecting an increase in his insulin as he's been on 3 units for a while but that one day I gave him 3.25 has sent his numbers all over the place. Doesn't make sense to me and now I don't dare go up to 3.25 as he's in the greens more than would be safe but I think he needs to go up.

I think if you can get a test @+6 & see where he is then you might get your answer. Panzer's nadar is anywhere between +4 & sometimes the next shot time. I caught him @47/45 retest @+7.5 the other day, which that's one drop towards a reduction. Two more & he goes down to 2units. I think we might see # 2 tonight. Fingers & paws crossed :cat:

I see you got him up to 58 but he dropped to 37. I not sure how much you should reduce to for AMPS. Let me see who's on & I will get back to you. I would try to get him into a bit safer #s before you go to bed.

We are getting through it. I have to go to work in about 3-4 hours so I may not be able to give him his full insulin if the numbers have not come up on there own over the next few hours as I need him to be safe.

Good job Claire. I know all about not getting enough sleep. I've learned to get broken sleep for over a year now. It's no fun,but it's what I have to do alot. Hopefully he surfs safely for you today.:bighug:
 
I'm wacked out as I did not sleep the day before either. He seems fine but sadly I have to go in this morning I probably also need to start a new post which I'll do later today.

I know he was being picky about food, when that happens I would err on the side of giving even higher carbs.

Typically, with anything in the 40s you'd give 1-2 tsp high carb (over 15%) and keep testing every 30 minutes. Wet food is ideal because it works more quickly, the kibble can take an hour or more. Below 40, you give 1-2 tsp high carb and drop or two of honey/karo and test every 15-20.
 
I wasn't on last night so didn't respond to the tag. A better option if you are not getting responses in time is to post over on the
Lantus / Levemir / Biosimilars forum. All the people who responded to you here also monitor there, and it'd give you even more eyes.

Paws crossed for that new dose.
