How to handle a lower than normal preshot number:

Until you collect enough data to know how your cat will react, we suggest following the guidelines in the FDMB's FAQ Q4.4:
Q4.4. My cat's pre-shot level was way below the usual value. Should I give the injection?
A4.4. There's no hard and fast rule, but if you don't have data on how your cat responds to insulin, here are some general guidelines.
  • Below 150 mg/dl (8.3 mmol/L), don't give insulin.
  • Between 150 and 200 (8.3-11.1 mmol/L), you have three options:
    • a.) give nothing
    • b.) give a token dose (10-25% of the usual dose)
    • c.) feed as usual, test in a couple of hours, and make a decision based on that value
  • Above 200 (11.1 mmol/L) but below the cat's normal pre-shot value, a reduced dose might be wise.
  • In all cases, if you are reducing or eliminating insulin, it's wise to check for ketones in the urine.
  • Above the normal pre-shot value, give the usual dose, but if the pre-shot value is consistently elevated, it's a good idea to schedule a full glucose curve to see whether a change in dose or insulin is appropriate. In most cases, the target "peak" value should not be below 100 mg/dl (5.6 mmol/L), and for some cats it might be higher.
Keep in mind these are general guidelines, and they should be personalized to your own cat's response to insulin. If your experience is that your cat does not became hypoglycemic with a dose which is close to her usual, then personal experience should be your guide.

When you get a low preshot next time, STALL DO NOT FEED and test again in 20-30 minutes to see if on the way up. Can you monitor if you shoot anything? Even with a token dose the first half of the cycle might be active requiring more testing. Do you have hc, mc, and lots of test strips?
 
I did. It’s been 40 minutes and it’s 101. Maybe I should pass on the insulin and do a curve? Every 2 hours test?

I have a relion and plenty of rest strips, at least 60. Thanks for the info. He was wining for food for a few hours and I didn’t realize I should wait but now I know
 
Skip or shoot 1.0 but only if you can test as much as needed. I do expect a bounce. If you shoot your night shot will be 11 1/2 hours from now and you can make up another 15 tomorrow morning. What do you want to do? No need for a curve but do get a +2 and you can go from there if you skip .

Thanks. I’ll skip and see what the PMPS is and then we can go from there. I’ll test again in 2 hours and maybe one this afternoon
 
That’s fine. A reduction was not earned since he didn’t go under 90. Let’s see the pmps.
Thanks so much. I just want to get him controlled before our trim in September. I’m interviewing a pet sitter tomorrow. She’ll come twice a day and give him his shot and good. She’s been trained and will test before food and shot. Hopefully she works out.i have 4 weeks to get him controlled
 
Your signature says SLGS. Please change it. I thought you were changing but the signature said otherwise. With TR you shoot if over 50 and reduce if under 50. So this was t close to a reduction.
 
Sharon, can you also update your spreadsheet, where it says "Dosing Method (SLGS or TR):" can you put "TR".

Also, your signature indicates you're using Lantus but your spreadsheet still says ProZinc.

Not surprised with the PMPS tonight with the skipped shot but you also now have some data as to what happens when you skip.

An important thing to remember with Lantus...you're not really shooting the number he is now. The shot doesn't usually start to "kick in" for 2-3 hours so you're shooting the number he'll be at in 2-3 hours. You'll also be feeding which will bring his numbers up. As long as you have plenty of supplies and can test as often as may be necessary, you might want to consider going ahead with the scheduled dose next time....get a +1 and +2 if you shoot a lower Pre-shot so if you need to intervene, you can do it early.

Nice cycle today....especially considering the skip yesterday.

Remember to start a new thread each day here. Otherwise people tend to get lost because if the thread gets really long, people figure there's nothing new going on and any questions/problems have already been taken care of. Even if you really don't have anything specific to say, if you post daily, people will learn more about you and your kitty. One of my first posts was about China playing with a cricket most of the night!.....then she ate it.....LOL
